- Background "SUBJECT: Lease washing machines and dryers at Wright Patterson Air Force Base, Ohio DESC: BACKGROUND: PLEASE GO TO www.pixs.wpafb.af.mil TO VIEW COMPLETE SYNOPSIS/SOLICITATION. Wright-Patterson AFB, OH has a requirement to lease commercially rated front- load washers, dryers, and stackable washers and dryers. This is a combined synopsis/solicitation for commercial services as supplemented with additional information included in this notice. This item prepared in accordance with (IAW) the format in FAR Subpart 12.6, announcement constitutes the only solicitation; quotes are being requested and a written solicitation will not be issued. Solicitation #F33601-01-T- 0512 is a Request for Quote (RFQ). This solicitation document and incorporated provisions and clauses are those in effect through Federal Acquisition Circular 97-19. This acquisition is restricted to 100% small business set- aside. The standard industrial classification (SIC) code is 7215. This acquisition has one line item for the base year and one line item for each option contract period: The line item is for the contractor to furnish all labor, parts, transportation and equipment to install, maintain, relocate, and repair Contractor-owned washers and dryers in specific locations on Wright-Patterson AFB, OH. Installation shall be completed by 03 Oct 01. The period of performance is 1 Oct 01 -- 30 Sep 02. Four one-year options may be awarded. Quotes will be provided for each year; (the base and optional years), totaling 5 years. All work will be IAW the attached Statement of Work (www.pixs.wpafb.af.mil) Solicitation provision at FAR 52.212-1, Instructions to Offerors- Commercial, (Oct 1995) is hereby incorporated by reference. The offeror shall provide past performance information on three recent and relevant contracts for the same or similar service. The offeror shall submit a Present/Past Performance Reference List citing the contract number, name, phone number and fax number of point of contact, and dollar value of contract. The Offeror will submit a copy of the Present/Past Performance Questionaire (attached to the website) to each of the three references with instructions to send completed questionaires to the Contracting Specialist Not Later Than 06 Aug 01. The address label shall reflect the following information: Solicitation Number F33601-00-T-0512, "To be delivered unopened to the Contracting Specialist (Mary Piper)" and shall be delivered to proposal address above. Past Performance questionaires may also be faxed to (937) 257- 3926 (Attn: Mary Piper). FAR 52.212-2 Evaluation Commercial Items; Award will be based on overall best value. Evaluation criteria to include price, past performance, and quality. Price will be submitted by cost per month to provide all work IAW the attached Statement of Work. Offeror will fill in the attached "information only" spreadsheet providing individual unit prices. This document shall be included as part of a complete RFQ package. All offerors shall include with their offers a completed copy of provision 52.212-3, Offeror Representations and Certification, Commercial Items. Clause 52.212-4, Contract Terms and Conditions, Commercial Items (Oct 95), is hereby incorporated by reference. The following additional FAR clauses cited in FAR 52.212-5, Contract Terms and Conditions Required to Implement Statutes or Executive Orders, Commercial Items are applicable to the acquisition: 52.222-26, Equal Opportunity; 52.222-35, Affirmative Action for Special Disabled and Vietnam Era Veterans; 52.222-36, Affirmative Action for Workers with Disabilities; 52.222-37, Employment Reports on Special Disabled Veterans and Veterans of Vietnam Era. The following FAR clauses, are incorporated into this contract by reference to implement provisions of law or executive orders: 52.212-41, Service Contract Act; 52.222-42, Statement of Equivalent Rates for Federal Hires; 52.222-43, Fair Labor Standards Act and Service Contract Act-Price Adjustment (Multiple Year and Option Contracts); 52.203-3, Gratuities; FAR 52.217-8, Option to Extend Services; FAR 52.217-9 Option to Extend the Term of Contract; Hazardous Material Identification and Material Safety; 52.232.25, Prompt Payment, are hereby incorporated by reference. The following FAR clauses are incorporated into this contract reference to implement provisions of law or executive orders: 52.222-26, Payment by Electronic Funds Transfer-Central Contractor Registration; 252.204-7004, Required Central Contractor Registration; 52.219-6, notice of Total Small Business Set Aside.
