The 325th Contracting Squadron at Tyndall AFB FL intends to award a purchase order pursuant to FAR 12.6 for the lease of a Computerized Bulk Pill Dispensing System. This is a combined synopsis/solicitation for commercial services prepared in accordance with the format in Subpart 12.6, as supplemented with additional information included in this notice. This announcement constitutes the only solicitation; proposals are being requested and a written solicitation will not be issued. This is a Request for Proposal (RFP), RFP No. F08637-99-R0005. The notice incorporates the Federal Acquisition Regulation (FAR) and DOD FAR Supplement (DFAR) provisions, and clauses in effect through the Federal Acquisition Circular (FAC) 97-13 and Defense Acquisition Circular (DAC) 91-13. This solicitation is unrestricted. This solicitation is open to all businesses under SIC 7352, size standard $5 million. The contractor shall furnish all labor, tools, parts, equipment, materials, supplies, personnel, transportation and other items necessary to install the system. The system should comply with the following: (1) Support a high volume of outpatient prescriptions (1,000/day); (2) Must interface with the Composite Health Care System (CHCS); (3) All counting cells must be movable to cutomize cell position; (4) Each unit must have readout capability that informs the user of the exact number of tablets/capsules that have been counted into the delivery chute; (5) Manufacturer will be responsible for perpetual maintenance and relocation of all units. Relocation must be done within thirty (30) days notice; (6) Must have display window so the user can see the drug inside the unit; (7) Must identification tag so the user can list manufacturer, lot # and the experation date (JCAHO requirement). See attached Statement of Need (SON) for the quantities requested and other services for Tyndall AFB FL. The 325th Medical Group at Tyndall AFB FL shall obtain from the contractor all lease services specified in the schedules that are required to be purchased during the applicable performance period. The anticipated purchase order will be bilateral and is expected to consist of a performance period which will not exceed the basic year, 30 Sep 99 -- 29 Sep 2000, Plus 4 option years (Option year 1: 30 Sep 2000 -- 29 Sep 2001; Option 2: 30 Sep 2001 -- 29 Sep 2002; Option year 3: 30 Sep 2002 -- 29 Sep 2003; Option year 4: 30 Sep 2003 -- 29 Sep 2004.). The offeror shall indicate a seperate line item for each option year showing monthly charge, and extended amount for the full option year, see attachment 1. Offerors are encouraged to submit commercial literature describing their operations and capabilities. OFFERORS MUST PROVIDE INFORMATION ON THEIR PAST PERFORMANCE, TO INCLUDE CONTRACT OR ORDER NUMBERS, REFERENCES (NAMES) AND PHONE NUMBERS. OFFERORS MUST ALSO SUBMIT A TECHNICAL PROPOSAL describing procedures. Contractor shall employ best commercial practices in all services performed. The contractor shall comply with all base security regulations and policy. The Government may conduct pre-award surveys to determine offerors_ technical and financial abilities to perform. The following clauses apply to this acquisition and are hereby incorporated by reference unless stated otherwise: FAR 52.212-1, Instructions to Offerors-Commercial; FAR 52.212-2 EVALUATION_COMMERCIAL ITEMS (in full text, see attachment 1); FAR 52.212-3, Offeror Representation and Certifications_Commercial Items (in full text, see attachment 1); FAR 52.212-4, Contract Terms and Conditions _ Commercial Items applies to this acquisition. It is tailored as follows: subparagraph (c) is changed to read: _Changes.
