Description

Medical Surveillance Examinations for full time support personnel of the New York Army National Guard. The USPFO-NY is requesting proposals for the performance of the subject medical exams at locations throughout New York State. Locations and estimated n umber of personnel at each location are as follows: 321 Manor Road, Staten Island, NY 51 personnel, 201 Schaeffer Drive, Ronkonkoma, NY 46 personnel, 330 Old Niskayuna Road, Latham, NY 78 personnel, Fort Drum, Watertown, NY 133 personnel, 184 Connecticutt Street, Buffalo, NY 22 personnel 6900 Thompson Road, Syracuse, NY 31 personnel, Camp Smith, Peekskill, NY 71 personnel, and 250 Weidner Road, Rochester, NY 97 personnel. Total estimated number of personnel is 575. The dollar magnitude of the solicitatio n is estimated at between $25,000 and $100,000. The solictation is a total small business set-aside with an applicable NAICS Code of 621999, with a small business size standard of $7,500,000. The anticpated issue date of the solicitation is 7 January 20 02 with a due date of on or around 7 February 2002. The proposed contract is a firm fixed price type with the award to be made to a single contractor who submits the lowest overall price in response to the solicitation.
