- N00244-02-R-0014 Amendment 0001 Medical Exams for Reservist Modification/Amendment 0001 to combined Synopsis/RFP N00244-02-R-0014 dated 19 Nov 2001. The purpose of this amendment is to add evaluation criteria to the above numbered Synopsis/RFP, provide answers to previously submitted questions and extend the closing date of the RFP from 14 Dec 2001, to 27 Dec 2001. 1. Offeror's shall propose time frames for delivering physical and laboratory results. Offeror's Past Performance will be evaluated in the area of contract delivery in order to substantiate claim of proposed delivery. Provision 52.212-2 is hereby amended to include the above delivery information. QUESTIONS AND ANSWERS: 1. Can you provide, or where can we obtain copies of all government forms that will be utilized in completion of this contract and what are the form numbers involved? Answer : This is a commercial acquisition. The government will accept standard commercial forms that address all of the points listed in the combined synopsis and further clarified herein. 2. Will the examinations be performed at the contractor's location? If not, where will they be held? Answer: Physical Exams will be performed at the vendor's location. 3. Will the examinations be batched or scheduled on a flow basis, as they are needed? Who schedules the examinations, the individual or the government? Answer: The Physical will be on a flow basis (as needed). Applicants who are eligible require physicals. The Recruiter will schedule the Physical. 4. What hours and days of the week will the contractor be required to be available. Answer: Monday through Sat. 07:00 am through 11:00 pm 5. What are the turnaround requirements for the examinations? Fo r example, from date of contact to appointment date, from date of appointment to date government must receive report, how many days for each. Answer: Physical exams require immediate transcription and submission. If labs are required, normal commercial industry standards should be proposed. Note: See change to evaluation criteria. 6. Will we be utilizing our own lab or the government's contracted lab? Who will compensate the lab and if the contractor is required to compensate the government contracted lab, at what rates? Answer: Yes you will utilize your own lab. 7. Who is the current contractor and what are the current costs for performing these exams? Answer: The government does not currently have a contract in place to any one specific contractor. The government has in the past utilized health care firms such as HealthSouth and HealthWorks. There i s no cost data available for release under this competition. 8. What are the historic frequencies for tests required for specific job specialties and as needed exams such as mammograms color vision, pregnancy and depth perception? Answer: These are initial test and the applicant once accepted into the Naval Reserve will utilize Naval Medical Facilities. 9. Do we bid a fixed price for each potential component or do we bid a price based upon each of the 3 categories of examinations? Answer: The government would expect to see a fixed price for each unit identified under each Category I-III. 10. For the over 40 male examinations, will we be expected to perform prostate exams and PSA lab tests as are often required under other similar military contracts? Answer: No. 11. We have two locations in San Diego County (and several others in the surrounding coun ties) will the government require that service be provided outside the metro San Diego area? Answer: Yes we require service throughout California and Hawaii. 12. How are these services outside the San Diego area provided? Answer: Services are provided on an as needed basis and payment is made via the government purchase card. The aforementioned vendors are utilized. 13 When do you anticipate award of this contract and when would you expect commencement? Answer: The government desires a contract award by 01 Jan 2002. The successful offeror will be required to be a full performance at contract award.
