- Description
- This is a combined synopsis/solicitation for commercial items prepared in accordance with the format in Subpart 12.6 as supplemented with additional information included in this notice. Proposals are being requested. The solicitation may also be viewed a t http://www.uspfo-mo.org. The solicitation number is DAHA23-02-T-0005. Solicitation document and incorporated provision and clauses are current through Federal Acquisition Circular 97-27. This request is being solicited on an unrestricted basis with SIC code 4812. The Missouri National Guard desires a firm-fixed price contract for leasing pagers with voice and callback number capabilities. Government desires 26 pagers with the following minimum requirements: (l) Operate nationwide; (2) All pagers will have local area numbers assigned; area code for Unit is (573); (3) Vendor will provide a detailed monthly statement for all pagers; (4) Vendor will provide a toll free number to resolve problems or to ask questions with a point of contact from Company; (5 ) Pager service 24-hours a day, 7-days a week; (6) Vendor will provide information on warranty, maintenance and or replacement information. Government will lease for 1-year period with option for an additional 4 one-year periods. Government may elect, wi th a 30-day written notice, to dispense with pager service without accruing penalties. Vendor will provide model and brand name of pager with proposal. The Government reserves the right to conduct discussions if determined by the Contracting Officer to b e necessary. Should discussions be deemed necessary because of proposal deficiency or other issues, they will be conducted either orally or in writing, only with those offerors determined to be in the competitive range. The offeror will be given the oppo rtunity to correct those deficiencies and resubmit their offer. The Government will select the most technically acceptable, low cost offer, and award will be made based on the best value to the Government. Vendor will provide with proposal a breakdown of cost. That is, rate per month within Missouri, rate for nationwide service per month, monthly cost for leasing pager, and total monthly cost for a quantity of 26 pagers. Offeror will submit (l) Technical proposal addressing ability to provide service; (2) Cost proposal as addressed above; (3) Past Performance, Evaluation Commercial Items FAR 52.212-2. (Submit three references of similar service which includes company name, point of contact and telephone number.) The following clauses apply: FAR 52. 212-1, Instructions to Offerors Commercial Items; Include a completed copy of the provision at FAR 52.212-3, Offeror Representation and Certifications-Commercial Item. FAR Clause 52.212-4, Contract Terms and Conditions; FAR Clause 52.212-5 Contract Terms and Conditions Required to Implement Statues or Executive Orders Commercial Items, 252.225-7001 and 252-225-7004.
