Description

This is a combined synopsis/solicitation for commercial items prepared in accordance with the format in Subpart 12.6, as supplemented with additional information included in this notice. This announcement constitutes the only solicitation; proposals are b eing requested and a written solicitation will not be issued. The solicitation number is DAKF06-02-T-0006 and is issued as a request for quotation (RFQ). The solicitation document and incorporated provisions and clauses are those in effect through Feder al Acquisition Circular 97-27. The anticipated contract is for one base year and one option year. This will be a 100% small business set aside (NAICS: 441229, size standard $5.0M). Responses should be quoted in accordance with the following: CLIN 0001, Quantity 12 months. Lease of three Hyster S50XM or equivalent. Contractor shall provide lease of three 4,000 lb forklifts with lp-gas powered engine. Indoor/outdoor usage; solid tires; propane power; three stage vista mast height; minimum 1 89 inch lift height. Contractor responsible for preventive maintenance, tire repair/replacement; vehicle repair within 24 hours of call in; vehicle replacement within 24 hours. Servicing must be accomplished during normal duty hours: Mon-Fri, 0630-1700 M ountain Time. Price shall include delivery/pickup charges to Ft Carson, CO. Period of performance: 01 December 2001 30 November 2002. CLIN 0002, Quantity 12 months. Delivered propane to run the forklifts. Usage estimated at 15 tanks per week (5 tanks/forklift/week). Period of performance: 01 December 2001 30 November 2002. CLIN 1001, Option year 1, Quantity 12 months. Lease of three Hyster S50XM or equivalent. Usage estimated at 15 tanks per week (5 tanks/forklift/week). Period of performance: 01 December 2002 30 November 2003. Delivery will be to: 64TH FSB, BLDG 8030, Fort Carson, CO 80913-4404 by 01 December 2001. Delivery will be FOB Destination. The provision at FAR 52.212-1 Instructions to Offerors Commercial Items and FAR 52.212-4 Contract Terms and Conditions are applicable to this solicitation. Offerors shall include with their offers a completed copy of the provision at FAR 52.212-3 Offeror Representations and Certifications Commercial Items (Alt I & III). Clause 52.212-5, Contract Terms and Conditions Required to Implement Statutes or Executive Orders Commercial Items (02/2000) is hereby incorporated by reference.
