Description

This amendment, N00259-02-T-0009-0002, is hereby issued as a combined synopsis/final resolicitation of N00259-01-T-0056. This final resolicitation is issued as a request for quotations (RFQ) for commercial items, prepared in accordance with the format in FAR subpart 12.6, as supplemented with additional information included in this notice. Quotations are being requested from open market and Federal Supply Schedule (FSS) contractors and a written solicitation will not be issued. Consistent with FAR 12.301(c), award will be made based on price , past performance, availability, and material conformance to the requirements as specified. These evaluation factors have equal levels of importance in the award determination. The solicitation document and incorporated provisions and clauses are those in effect through FAC 97-27. This is a requirement for surgical scrubs. Item 0001: SCRUB SHIRTS with the following essential physical and functional characteristics: V- neck, bartacked. This acquisition incorporates the following FAR provisions and clauses: 52.212-1 INSTRUCTIONS TO OFFERORS-COMMERCIAL ITEMS(Oct 95)consistent with customary commercial practices in the surgical scrubs industry; 52.212-3 OFFEROR REPRESENTATIONS AND CERTIFICATIONS-COMMERCIAL ITEMS (OCT 95) must be completed and submitted along with the quote. The following additional information is made part of this final resolicitation: (1) Item 0001: White block lettering on one side using the kaumograph or heat transfer process; (2) Consideration will be given to open market quotations from small and woman-owned businesses consistent with FAR Part 19; (3) The small business set-aside requirements do not apply to quotations received from FSS contractors; (4) Colorfast applies to industry-accepted standards. The Government will not conduct colorfast testing; (5) Consideration will be given to brand name (Fashion Seal) or equal; and (6) This final resolicitation closes at 4 p.m., Pacific Standard Time, on November 21, 2001.
