- Description
- Provide Home Infusion Therapy services, as specified, in the homes of eligible veterans who are designated by, and are living within the service area of the VA Medical Center, Spokane, Washington. Contract performance will be in accordance with this Request for Proposal (RFP) and the offer that is accepted by the VA. Referrals generated from the Spokane Veterans Affairs Medical Center (SVAMC) payment source will depend on eligibility and will be coordinated through the Community Based Services Team (CBST) staff of the Patient & Community Care Service. If payment source is other than SVAMC, i.e. Medicare, Public Aide, Private Insurance, Champus, Health Management Organization (HMO), Preferred Provider Organization (PPO), the CBST will act as the liaison for SVAMC. This contract will delineate reimbursement rates when medications, supplies and/or nursing care is paid directly by SVAMC. The SVAMC will determine eligibility of the beneficiary through the Home Care Program, the attending physician and the Interdisciplinary Team. These include orders of patient admission and discharge to the program. Contractor Service Shall Include: Pharmacy Support, Nursing, Education, Quality Management, On-Site Inspection, Laboratory, Hospitalization Of Veteran, Assist In Relocation Of Veterans (if needed). NAICS Code 621610 size standard 10-million. Estimated dollar is $25,000 to 100,000. Interested parties should contact Michael C.
