Description

The South Texas Veterans Healthcare System, Audie L. Murphy Division, has a requirement for surgical medial devices used in angioplasty procedures. The Audie L. Murphy VA Hospital, San Antonio, TX, proposes to award a sole source requirements contract for 1 base year and (4) one year options for renewal to Boston Scientific/Medi-tech Inc. Vendor is to provide various angioplasty balloons, Vena Cava filters, guidewires, introducer sheaths, guiding catheters, thrombectomy devices, and metallic stents. This agreement will contain tier pricing based on overall hospital usage of products of like description and useage. Usage levels will be evaluated semi-annually to ensure proper tier pricing discounts are being provided. This procurement is being conducted in accordance with Federal Acquisition Regulation subpart 13.501(a)(2)(I), Test Program, using simplified acquisition procedures. Interested persons may identify interest and ability to respond to this requirement. Information received will normally be considered for the purpose of determining whether to make a multiple award. The Government intends to make a single award, however reserves the right to make multiple awards. Request for Quote package may be obtained by mail, email, or fax to the above office referencing RFQ 671-16Q-02. Faxed requests can be submitted at 210-617-5255. All responsible sources must submit a Request for Quotation to be considered. All Request for Quotes must be submitted no later that 5:00pm, CST, December 4, 2001.
