Description
GENERAL STATEMENT OF WORK =96 LICENSED PRACTICAL NURSE/REGISTERED NURSE (LPN/RN) SERVICES. The Contractor shall provide LPN/RN services to perform nursing services in the VA patient=92s home. All services shall be performed at the patient=92s home in Clinton County, New York. The Contractor=92s personnel must be present at the patient=92s home for the performance of any services required during the contract period. LPN/RN services shall include but not limited to: Accurately assess patient biopsychosocial needs and document appropriately on the Nursing Assessment Flow sheet and/or progress notes. Effectively plan patient care through use of standards/protocols and modify care based on changes in patient=92s status. Provide care within the scope of typical acute and chronic care standards (i.e. Tracheotomy tube, G-tube feeding, skin care. Intervene effectively in emergency situations.=20 Use of AMBU bag for backup if generator not available. Learn how to use generator when it becomes available. Evaluate patient=92s response to treatments. Coordinate care through effective communication with the interdisciplinary team. Demonstrate effective organizational, decision-making, and interpersonal skills. Communicate via telemedicine equipment, telephone, and agency reports. LPN/RN shall provide personal care of patient including feeding, oral hygiene, shaving, skin care (bathing), showers/tub baths, application of external catheters/drainage bags, bed making, distribution of nourishment. Universal Precautions should be observed as per agency policy. LPN/RN shall perform, report, record vital signs, neurological vital signs, pulses, intake and output measurements, cleansing and retention enemas, suppository placement, digital rectal stimulation, urethral catheterization PRN, tube feeding (gastrostomy), sterile and non-sterile specimen collection, range of joint motion, elastic bandage and hose application, hot and cold non-sterile compresses, sterile dressing and irrigations, ventilator care and management. Oral, nasal, and tracheal suctioning, oxygen therapy, and understand how and when to utilize a generator. LPN/RN shall observe and report changes/abnormalities in patient condition. Assist with modification of treatment plans to accommodate cultural preferences, religious preferences, and changes that occur with aging. Assist in formulating nursing objectives for individual patient care. Participate in the implementation of the care plan developed. Support the health care team (e.g. MD, OT) in diagnostic and therapeutic activities. =20 Initiate appropriate rehabilitative measures (ROM). Reinforce relevant patient/family education activities. Recognize and initiate appropriate actions in urgent/emergent situation, including cardiopulmonary resuscitation (BLS) when appropriate, as per patient wishes. Utilize appropriate emergency evacuation techniques. =20 SPECIAL CONTRACT REQUIREMENTS QUALIFICATIONS: The Contractor=92s personnel assigned to supervise the nursing staff performing services under this contract must be a Registered Nurse (RN). Personnel assigned by the contractor to perform the services must be licensed Registered Nurses or Licensed Practical Nurses in State, Territory, or Commonwealth of the United States or the District of Columbia. The qualifications of such personnel shall be subject to review by the VA Nurse Manager, Executive Physician, and approval by the Medical Center Director. Documentation of current licensing of each RN and LPN on duty must be provided to the VA and will be verified as to it authenticity. The Contractor will be responsible for providing all copies of license renewals. There must be evidence in the Contractor=92s files that an in-depth interview took palce between the Contractor=92s staff RN interviewer and each contract employee. Documentation must be provided proving each contract employee has participated in annual safty, hazardous waste, and infection control training. The contractor must have on file, documentation to prove the contract nurse hired and sent to the VA, has at least one year, within the last 5 years, successful professional nursing experience in the area to which they are assigned to work. A minimum of two satisfactory references of performance as an RN or LPN must be on record. Documented physical exam, which includes Drug and Alcohol history. QUALITY ASSURANCE STANDARDS FOR CONTRACT SERVICES. The Contractor shall develop a quality control program to insure that RN/LPN services are performed in accordance with existing Federal, state, and local guidelines; and within existing policies and procedures of the VA Medical Center. =09 SUPERVISORY CONTROL. Contractor=92s personnel shall work under the supervison of a professional registered nurse on any tour of duty.=20 Contractor=92s personnel shall be expected to perform routine work on their own initiative and seek RN guidance when the situation demands.=20 Assignments may be oral or written and work will be spot checked.
