- This is a combined synopsis/solicitation for commercial items prepared in accordance with the format in FAR subpart 12.6 as supplemented with additional information provided in this notice. THIS NOTICE IS THE SOLICITATION DOCUMENT. An quote is being requested and a written solicitation will not be issued. Solicitation F22600-02-R-0001 is 100% set aside for Small Business concerns. The solicitation document and incorporated provisions and clauses are those in effect though Federal Acquisition Circular 97-27. Keesler AFB intends to issue a firm-fixed price contract for a base period of 10 months (01 Dec 01-30 Sep 02) and 4 one-year option periods (1 Oct 02-30 Sep 06) for the lease (with option to buy) of two (2) PharmASSIST ES/G-314 systems for the Keesler Medical Center Pharmacy Services. Each system to consist of 3 PharmASSIST 50-unit Standard Cabinets, 4 workstations and all required software. =20 Performance period U/I Qty Unit price Extended cost 01 Dec 01-30 Sep 02 MO 12 $ $ =09 01 Oct 02-30 Sep 03 MO 12 $ $ =09 01 Oct 03-30 Sep 04 MO 12 $ $ =09 01 Oct 04-30 Sep 05 MO 12 $ $ =09 01 Oct 05 ? 30 Sep 06 MO 12 $ $ =09 The North American Industry Classification Code is 532490 and the small business standard is $5 million. This is a DO rated order with a priority identification symbol of C9E. The following Federal Acquisition Regulations (FAR) provisions and clauses apply: FAR 52.212-1 Instructions to Offerors-Commercial Items is amended to read: submit signed and dated offer on Company Letterhead to 81 CONS/LGCB , 310 M Street, Room 102, Keesler AFB MS 39534-2147 at or before 1530(3:30pm) CST 13 November 01. Quotation may be submitted via FAX (228) 377-3298 or e-mail to tom.kocurek@keesler.af.mil . =20 Each offeror shall include a completed copy of the provision at FAR 52.212-3 -- Offeror Representations and Certifications?Commercial Items, with their quote. FAR 52.212-4 Contract Terms and Conditions?Commercial Items, is incorporated by reference. The following paragraphs of this clause are tailored as follows:(a) Inspection/Acceptance. Inspection and acceptance of the services will be performed at Keesler Air Force Base, MS, by 81 MDSS/SGSLC.; (c) Changes. Changes in the terms and conditions of this contract may be made only by written agreement of the parties, with the exception of administrative changes such as changes in the paying office, appropriations data, etc., which may be changed unilaterally by the Government. FAR 52.212-5 Contract Terms and Conditions Required to Implement Statutes or Executive Orders?Commercial Items (Deviation) and DFARS 252.212-7001 (Deviation) are incorporated, however for paragraph (b), only the following clauses apply: FAR 52.222-26, Equal Opportunity; FAR 52.222-35 Affirmative Action for Disabled Veterans and Veterans of the Vietnam Era, and FAR 52.222-36 Affirmative Action for Workers with Disabilities. The following clauses are also incorporated by reference: FAR 52.204-6 Data Universal Numbering System (DUNS) Number; FAR 52.217-8 Option to Extend Services (For the purpose of this clause, the Contracting Officer may exercise the option by written notice to the Contractor within 15 calendar days prior to the expiration date of the contract. (b) The date mailed or otherwise furnished to the Contractor will be considered notification.); FAR 52.217-9 Option to Extend the Term of the Contract (For the purpose of this clause the blanks are to be completed as follows: (a) Within 15 calendar days prior to the expiration date of the contract. (c) Not to exceed 58 months. (d) The date mailed or otherwise furnished to the Contractor will be considered notification.); FAR 52.225-13 Restrictions on Certain Foreign Purchases; FAR 52.232-18 Availability of Funds; FAR 52.232.33 Payment by Electronic Funds Transfer-Central Contractor Registration; DFARS 252.204-7004 Required Central Contractor Registration, and DFARS 252.225-7001 Buy American Act-Balance of Payments Program Certificate; and AETC FARS 5352.217-9000 Option Clause Limitation Notice.=20 The Government intends to award a firm-fixed price contract based on price, technical capability of proposed systems to meet requirements and past performance. Technical capability is more important than price and past performance combined. Include with quote at least 2 references of past performance to contact.
