Description

This is a combined synopsis/solicitation for commercial items prepared in accordance with the format in Subpart 12.6, as supplemented with additional information included in this notice. This announcement constitutes the only solicitation; proposals are b eing requested and a written solicitation will not be issued. The solicitation number is DAKF06-02-T-0005 and is issued as a request for quotation (RFQ). The solicitation document and incorporated provisions and clauses are those in effect through Feder al Acquisition Circular 97-27. The anticipated contract is for one base year and four option years. This will be a 100% small business set aside (NAICS: 441229, size standard $5.0M). Responses should be quoted in accordance with the following: CLIN 0001, Quantity 12 months, rental of 4 each 5000 lb lift fork lifts. The contractor shall provide four (4) 5K Electric Forklifts w/chargers. Each forklift must clear 120 inch height and 95 inch width. MAST 187 inch with 42 inch fork. Non-inflata ble rubber tires to be used on a flat, smooth surface. Maintenance warranty for normal services every quarter and for repairing flat tires and other mechanical problems within 12 hours. Period of performance: 01 December 2001 30 November 2002. CLIN 0002, Quantity 12 months, rental of 2 each 4000 lb lift fork lifts. The contractor shall provide two (2) 4K Electric Forklifts w/chargers. Each forklift must clear 120 inch height and 95 inch width. MAST 187 inch with 42 inch fork. Period of performance: 01 December 2005 30 November 2006. Delivery will be to: 3D ACR, 4652 Quinn, Bldg 2352 Fort Carson, CO 80913-4404 by 01 December 2001. Delivery will be FOB Destination. The provision at FAR 52.212-1 Instructions to Offerors Commercial Items and FAR 52.212-4 Contract Terms and Conditions are applicable to this solicitation. Offerors shall include with their offers a completed copy of the provision at FAR 52.212-3 Offeror Representations and Certifications Commercial Items (Alt I & III).
