Description
The department of Veterans Affairs medical Center, Birmingham Alabama requires a contractor to provide medical equipment and support equipment for a Cost Per Test Contract with an estimated 3,000 proceedures per year. The following are the requirement for this contract. 2. Requirements: The following are the minimum requirements for the equipment needed to perform the Endoscopy test procedures by the DVA Medical Center. All equipment associated with this contract shall be delivered new upon installation. (a) Two (2) stretcher chairs which feature easy chair to stretcher conversion, adjustable height, reverse Trendelenburg position capabilities, c-arm compatibility, chair shall be equal to models from Stryker, Olympus, etc. that have the above mentioned characteristics=94 (b) One (1) Pediatric Gastroscope with the insertion tube diameter of 6 mm to 9 mm, angle of view is a minimum of 120, channel size of 2.0 mm to 2.4 mm. This scope should permit the endoscopist to push the heater probe, biopsy forceps or other therapeutic accessories (i.e. sclerotherapy needle) while the endoscope in on a retroflexed view. (c) One (1) Pediatric Video Colonoscope, with an insertion tube diameter of 10-12mm and an up/down angulation of 180 degrees. The minimum field of view shall be 120 degrees, with a channel size of at lease 2.8 mm. This scope should permit the endoscopist to push the heater probe, biopsy forceps or other therapeutic accessories (i.e. sclerotherapy needle) while the endoscope in on a retroflexed view. (d) Four (4) Videoprocessors capable of generating patient reports. =20 (e) Four (4) OES Xenon Light Source, to support four (4) rooms. (f) Four (4) 19=94 minimum color video monitor, to support four (4) rooms. (g) Four (4) Endoscopy Procedure and Image Cart to accommodate a minimum size monitor of 19 inches, with 3 shelves, no less than 3 drawers (one lockable), and multiple outlet plug strip. This cart shall be customized to fit the equipment that we will be using. (h) Four (4) Video Sigmoidoscope, with a minimum field of view of 120 degrees, channel size of at lease 3.0 mm, and flushing channel or forward water jet. This scope should permit the endoscopist to push the heater probe, biopsy forceps or other therapeutic accessories (i.e. sclerotherapy needle) while the endoscope in on a retroflexed view. (i) Five (5) Video Gastroscopes with minimum field of view of 120 degrees, channel size of at least 2.8 mm, flushing channel or forward water jet. This scope should permit the endoscopist to push the heater probe, biopsy forceps or other therapeutic accessories (i.e. sclerotherapy needle) while the endoscope in on a retroflexed view. (j) Five (5) Video Colonoscopes, with a minimum field of view of 120 degrees, minimum channel size of 3.6 mm, and an additional independent flushing channel. This scope should permit the endoscopist to push the heater probe, biopsy forceps or other therapeutic accessories (i.e. sclerotherapy needle) while the endoscope in on a retroflexed view. (k) Two (2) Large Channel (Therapeutic) Video Duodenoscope, with a minimum field of view of 110 degrees, channel size minimum of 4.0 mm. The elevator shaft must be sterilisable or the shaft self-contained. (l) One (1) new Video therapeutic (i.e. two channel) gastroscope, with a minimum angle of view of 100 to 120 degrees. The therapeutic or working channel size must be at least 3.4 mm, and the other channel must have a minimum size of 2.8 mm. The therapeutic channel size must be wide enough to allow passage of the therapeutic heater probe, or 2 plastic catheters. Also large channels are required in order to be able to aspirate blood during cases of severe upper gastrointestinal hemorrhage. This scope should permit the endoscopist to push the heater probe, biopsy forceps or other therapeutic accessories (i.e. sclerotherapy needle) while the endoscope in on a retroflexed view. (m) Four (4) PC Workstations, Pentium 4 CPU operating at no less than 2 GHz with no less than 128 Megabytes RAM, and no less than 2 gig hard drive, and with Window's 2000 operating system. (n) Two (2) Workstation compatible with network dictation system, (o) One (1) Fax server for dictated faxing, (p) One (1) 10-User Server to include, Window's 2000 Server with 4 Intel Pentium lll Xenon Processors at 900 MHz and 512 Meg RAM. Endoscopy application software and hardware components including network software, tape backup, and uninterruptible power supply. (q) One (1) network system that will allow two physicians to dictate reports simultaneous, (r) One (1) Network Hub (12 Workstations), (s) One (1) Network Color Laser printer with no less than 32 MB memory upgrade capable of printing photo quality digital images. (t) Software link to enable completed endoscopy reports to be transmitted to the hospital's VISTA computer system's Computerized Patient Record System (CPRS) under the Reports tab of the patient chart for electronic signature authentication in VISTA. (u) All cables and hookups to ensure that all equipment performs as designed. 3. Other Computer Requirements: Security: The system must be configured so that it meets the following minimum standards: (1) User Identification, User access will be controlled and limited based on positive user identification and authentication mechanisms that support the minimum requirements of access controls. (2) To ensure accountability, individual access codes (passwords) are mandatory for all sensitive VHA information systems. (3) Controls must be implemented to require passwords that meets department standards and complexity such as described in VA CIO Council Policy dated January 21, 2000, which states "Controls shall be implemented to require strong passwords. Passwords shall be at least eight characters in length, and contain three of the following four kinds of characters: (upper case and lower case), numbers, and characters that are neither nor numbers like ("#", "@", or "$")" Electronic Signature: The system must allow for electronic signatures to be entered. This electronic signature will appear on computer generated record documents to authenticate and/or countersign medical records. The electronic signature will be treated as a written signature with all of the ethical and legal implications thereof.=20 Attending Doctors must be able to sign their electronic signature on the work that has been completed by a Fellow, so the both names are shown on the document in electronic signature form. Amending Reports: This system will allow VA authorized personnel the function of amending reports after they have been verified. According to the Privacy Act of 1974, medical record documentation must be able to be amended if found to be erroneous.
