- Subcription for Pharmacy Database and BioDatabase(TOMES) -This is a combined synopsis/solicitation for commercial items prepared in accordance with FAR Subpart 12.6. PLEASE READ THIS ENTIRE NOTICE CAREFULLY AS IT CONSTITUTES THE ONLY NOTICE THAT WILL BE ISSUED. This is a request for quotation (RFQ). The incorporated document provisions and clauses are those in effect through Federal Acquisition Circular 97-12. The Standard Industrial Classification (SIC) Code is 7375. The NAICS Code is 51140. PROJECT: 1. Subscription for Biodatabase: registered databases includes: MEDITEXT (medical Management)*; HAZARDTEXT (Hazard Management)*; CHRIS (Chemical Hazard Response Information System); HSDB (Hazardous Substance Data Bank); IRIS (Integrated Risk Information System); NAERG (North American Emergency Response Guidebood Documents); New Jersey Hazardous Substance Fact Sheets NIOSH Pocket Guide; OHM/TADS (oil and Hazardous Materials/Technical Assistance Data Systems); RTECS (Registry of Toxic Effects of Chemical Substances); REPROTEXT System; REPROTOX System; Shepard's Catalog of Teratogenic Agents; LOLI (Regulated Chemicals List of Lists). PROJECT: 1. Subscription for Pharmacy Database: registered databases includes: DRUGDEX, EMERGINDEX, INJURY/ILLNESS AFTER CARE INSTRUCTIONS, PHARMACEUTICAL MSDS, POISINDEX, PDR, USPDI AFTERCARE INSTRUCTIONS, REPRORISK, TOMES AND DRUGREAX. Delivery terms shall be FOB Destination, 3rd Medical Group 5955 Ziamer Ave, #505851 Bio-Environmental & #505610 Pharmancy, Elmendorf AFB, Alaska 99506. The following provisions and clauses are incorporated and shall remain in full force in any resultant purchase order; FAR Part 52.212-1 Instructions to Offerors-Commercial Items, is incorporated by reference. The provision at 52.212-2 Evaluation of Commercial Items, applies to this solicitation. The Government reserves the right to make award on the initial quote received without discussions. Each offeror shall include a completed copy of the provision at FAR 52.212-3 Offeror Representations and Certifications-Commercial Items with its quote. FAR Part 52.212-4 Contract Terms and Conditions-Commercial Items, is incorporated by reference and applies to this acquisition. Flag Commercial Vessels. FAR 52.212-5 Contract Terms and Conditions Required to Implement Statutes or Executive Order?Commercial Items (XXX 2001) (DEVIATION). Incorporate DFARS 252.212-7001 Contract Terms and Conditions Required to Implement Statutes or Executive Orders Applicable to Defense Acquisitions of Commercial Items. 252.225-7001 Buy American Act and Balance of Payment Program. DFARS 252.212-7001 Contract Terms and Conditions Required to Implement Statutes or Executive Orders-Commercial Items (XXX 2001) (DEVIATION). DFARS 252.222-7000 Restrictions on Employment of Personnel (Mar 2000)(IAW DFARS 222.7003). The contractor shall extend to the Government their standard warranty offered in similar types of sales. Acceptance of the warranty does not waive the Government?s right with regard to the other terms and conditions of the contract. In the event of a conflict, the terms and conditions of the contract shall take precedence over the warranty. The warranty period shall begin upon final acceptance of the items listed in the schedule. This is a DO rated order with priority identification symbol of C9E. A Firm-Fixed Price purchase order will be issued under the Simplified Acquisitions Procedures. Submit signed and dated offer to 3CONS/LGCB at or before 5November 2001 4:00PM Alaska Standard Time. Quotation shall be signed, dated and submitted on your company?s letterhead or quotation forms. Quotations may be submitted via fax. All questions concerning this amendment must be submitted in writing and faxed to A1C Veronica LaBouve at (907) 552-7496. No telephonic responses will be processed.
