Description
This is an amendment to the combined CBD/RFQ noticed posted on October 18, 2001 and printed on October 25, 2001 for lease or rental of facilities. The due date is extended to Monday, October 29, 2001. Amendment 001 is hereby issued to amend/add the following contract line item numbers (CLIN): CLIN 001: November 7-8, 2001 -- one (1) general meeting room to accommodate up to 325 participants (rounds of 10); CLIN 002: November 7 -- seventeen (17) breakout rooms to accommodate up to 20 participants (rounds of 10); November 8 -- three (3) breakout rooms to accommodate up to 100 participants and one (1) breakout room to accommodate up to 50 participants; CLIN 003: November 7-8, 2001 -- continental breakfast for approximately 325 participants; CLIN 004: November 7-8, 2001 -- light refreshments for one (1) morning and one (1) afternoon break for approximately 325 participants; CLIN 005: November 7, 2001 -- light refreshments for approximately 325 participants for award ceremony; CLIN 006: November 6, 2001, 12:30 p.m. -- 4:30 p.m. (SRM 101 Workshop) -- one (1) meeting room to accommodate up to 120 participants (rounds of 10) and head table w/three (3) chairs and (2) microphones; CLIN 007: audio visual needs for SRM 101 Workshop -- two (2) flipcharts w/stands and markers; (1) projection screen; (1) LCD projector; (1) podium w/removable microphone; CLIN 008: November 7-8, 2001 -- audio visual needs for general meetings session -- table and (8) chairs placed on stage for panel members; (8) microphones w/stands on panel members' table; (3) handheld cordless microphones; (8) lavalieres; (1) podium with microphone; (2) projection screens; (2) LCD projectors; registration tables w/10 chairs; (1) overhead projector; CLIN 009: audio visual needs for breakout rooms on November 7, 2001 -- (17) flipcharts w/stands and markers; and CLIN 010 -- audio visual needs for breakout rooms on November 8, 2001 -- (4) LCD projectors; (4) projection screens; (8) flipcharts w/stands and markers (2 per room). The hotel must also provide 245 sleeping rooms (reserved and guaranteed at the prevailing government rate; individually paid by IRS employees/guest).
