Description

This is a combined synopsis/solicitation for commercial items prepared in accordance with the format in FAR, Subpart 12.6, as supplemented with additional information included in this notice. This announcement constitutes the only solicitation; proposals are being requested and a written solicitation will not be issued. This solicitation is being issued as a Request for Quotation (RFQ) number 688-36-01. The solicitation document and incorporated provisions and clauses are those in effect through Federal Acquisition Circular number 97-27. The North American Industry Classification System (NAICS) for this solicitation is 541620. The small business size standard is 5 million dollars. This acquisition is set aside for small business only. Description of Work: Contractor to provide all necessary labor, parts, and equipment required to perform a re-inspection and identification of asbestos containing material (ACM) at the VA Medical Center. This is a re-inspection and follow-up of the original survey and report of the entire facility conducted in 1992. The 1992 report has been updated annually. The Contractor shall review the original report and the annual re-inspection reports, note any changes in ACM status/conditions since the last re-inspection and submit a detailed report, update the HAZCAD database and enter all information into the VAMC's computer. The inspection shall follow and conform to AHERA guidelines and local, federal and VA guidelines. Award of this contract is subject to the availability of funds and may be extended for two (2) additional option years in accordance with the clause at 52.217-9, Option to Extend the Term of the Contract. All work shall be performed by a Certified Industrial Hygienist. A copy of the hygienist certification must be submitted with the quote.
