Description

This is a combined synopsis/solicitation for commercial items prepared in accordance with FAR Subpart 12.6, as supplemented with additional information included in this notice. This announcement constitutes the only solicitation; quotations are being requested and a written solicitation WILL NOT be issued. Solicitation number RFQ-797-FDF4-02-0010 is issued as a Request for Quotation. The solicitation document and incorporated provisions and clauses are those in effect through FAC 97-27. The NAICS code is 422210 and the small business size standard is 100 employees. This action is set aside for small business. The Air Force Medical Logistics Office/Veterans Affairs Special Services (AFMLO/VASS) Contracting Activity has a requirement for Cefazolin Sodium Injection USP 1GM/GM Cefazolin 10ml vial 25's NDC# 00364-2465-34 for the medical treatment facility APO AP 96214. The contractor shall provide the following: quantity of 1333 boxes (25 vials per box). Delivery is FOB destination to APO AP 96214 no later than November 21, 2001. The following FAR clauses apply to this acquisition. FAR clause 52.212-1, Instructions to Offerors -- Commercial Items; FAR Clause 52.212-4, Contract Terms and Conditions -- Commercial Items; FAR Clause 52.212-3, Offeror Representations and Certification -- Commercial Items; and FAR Clause 52.212-5, Contract Terms and Conditions Required to Implement Statutes or Executive Orders -- Commercial Items. Offerors shall include a completed copy of the provision at 52.212-3, Offeror Representations and Certification -- Commercial Items, with the offer. The Government anticipates award of a purchase order resulting from this solicitation to the responsive/responsible offeror whose offer conforms to the solicitation and is most advantageous to the Government.
