- This is a combined synopsis/solicitation for commercial items prepared in accordance with the format in FAR subpart 12.6, as supplemented with additional information included in this notice. This announcement constitutes the only solicitation; proposals are being requested and a written solicitation will not be issued. This solicitation is issued as Request for Quotation (RFQ) No. F49642-02-T-0001. The solicitation document and incorporated provisions and clauses are those in effect through Federal Acquisition Circular (FAC) 97-26. This is a small business set-aside. ITEM NO SUPPLIES/SERVICES QUANTITY UNIT UNIT PRICE AMOUNT 0003 20.00 Hours ________ __________ =20 EMERGENCY INTERVENING SERVICE CALLS (OPPM) NON PERSONAL SERVICES: CONTRACTOR SHALL PROVIDE ALL PLANT, LABOR, TOOLS, PERSONNEL, EQUIPMENT, MATERIAL, TRANSPORTATION, SUPERVISION AND ALL OTHER ITEMS AND SERVICES NECESSARY TO PERFORM OUTSIDE PRINCIPAL PERIOD=20 MAINTENANCE (OPPM) FOR COMPUTERIZED CONTROL SYSTEM IN STRICT=20 COMPLIANCE WITH THE REQUIREMENTS AS SET FORTH IN THE CONTRACT=20 SPECIFICATION DURING OTHER THAN NORMAL DUTY HOURS. =20 _____________ =20 ITEM NO SUPPLIES/SERVICES QUANTITY UNIT UNIT PRICE AMOUNT 0004 01 Lump Sum ________ _________ REIMBURSABLE PARTS THE CONTRACTOR SHALL FURNISH ALL PARTS FOR THE CENTRAL HEAT=20 PLANT COMPUTERIZED CONTROL SYSTEM IN STRICT COMPLIANCE WITH THE REQUIREMENTS AS SET FORTH IN THE CONTRACT SPECIFICATIONS.=20 ____________ STATEMENT OF WORK SECTION C-1 GENERAL 1.1 Scope Of Work: The contractor shall furnish all plant, labor, equipment, transportation, and materials to perform all operations necessary to inspect, repair, and maintain controls at the Central Heat Plant (Bldg 18), Bolling AFB, in accordance with the specifications and subject to the terms and conditions of the contract. 1.2 Contractor Personnel:=20 1.2.1 Contract Manager The contractor shall provide a contract manager who shall be responsible for the performance of the work. The name of the person, and an alternate who shall act for the contractor when the manager is absent shall be designated in writing to the contracting officer. 1.2.1.1 The contract manager or alternate shall have full authority to act for the contractor on all contract matters relating to daily operation of this contract. 1.2.1.2 The contract manager or alternate shall be available during normal duty hours within 1 hour to meet on the installation with government personnel to discuss problem areas. After normal duty hours, the manager or alternate shall be available within four hours. 1.2.1.3 The contract manager and alternate or alternates must be able to read, write, speak, and understand English. 1.2.2. Contractor Employees. The contractor shall not employ persons for work on this =20 contract if such employee is identified to the contractor by the contracting officer =20 as a potential threat to the health, safety, security, general well being or operational=20 mission of the installation and its population. 1.2.2.1. Contractor personnel shall present neat appearance and be easily recognized. This=20 may be accomplished by wearing clothing bearing the name of the company or=20 by wearing appropriate badges, which contain the company name and employee=20 name. 1.2.2.2. Certification of Employees: The contractor shall furnish to the Contracting=20 Officer, a letter certifying that all those mechanics who will be performing=20 maintenance and repair work are qualified, experienced mechanics. The letter=20 shall contain a list of names indicating after each name a resume of each=20 particular mechanics experience and training. The Contracting Officer shall have =20 the option to further investigate the qualifications of each individual and=20 disallow any who do not meet with the approval of the Contracting Officer from=20 performing any work on these controls. 1.2.2.3. The contractor shall not employ any person who is an employee of the United=20 States Government if the employment of that person would create a conflict of=20 interest, nor shall the contractor employ any person who is an employee of the=20 Department of the Air Force, either military or civilian, unless such person seeks=20 and receives approval in accordance with DODD 5500-7-R (Joint Ethics=20 Regulation). In addition, the contractor shall not employ any person who is an=20 employee of the Department of the Air Force if such employment would be=20 contrary to the policies in AFI 64-106. 1.2.2.4. The contractor is cautioned that off duty active military personnel hired under=20 this contract may be subject to permanent change of station, change of duty=20 hours, or deployment. Military Reservists and National Guard members may be=20 subject to recall to active duty. The abrupt absence of these personnel could=20 adversely affect the contractor?s ability to perform; however, their absence at any=20 time shall not constitute an excuse for nonperformance under this contract. 1.2.3. Security Requirements: The contractor shall obtain and maintain for his=20 personnel, the following pass and identification items: Bolling Air Force Base, AF=20 Form 75 Vehicle/Entry pass, which shall be used for work and entrance to provide=20 maintenance and respond to service calls at Bldg. 18. 11CES/CEOEC will provide=20 request for AF Form 75 to 11 SPAP after obtaining names and verification of=20 employment from contractor. Lost vehicle and entry passes shall be reported=20 immediately to Security Police, and Quality Assurance Evaluator. 1.2.3.1. Employee Training: The contractor employees shall have experience in=20 servicing equipment listed in Section C-4 and with Bristol Babcock software=20 programming used in this application. Contractor shall be required to show=20 certification of work on this equipment before award. 1.3. Quality Control: The contractor shall establish and maintain a complete Quality=20 Control Plan to ensure the requirements of the contract are provided as specified.=20 One copy must be provided to the contracting officer on the contract start date and as=20 changes occur. 1.4. Quality Assurance: The Government will evaluate the contractor?s performance=20 under this contract. The government will record all surveillance observation. When=20 an observation indicated defective performance, the Quality Assurance Evaluator=20 will require the contractor?s representative initial the observation and require the=20 contractor to re-perform the work. 1.5. Hours of Operation:=20 1.5.1. Normal Hours: Work shall be performed during the normal workweek, 07:30 am=20 to 4:00 pm, Monday through Friday. Deviation from the normal workweek will not=20 be granted except under unusual circumstances. Request to work on Saturday,=20 Sunday, and/or Federal holidays shall be submitted to the Contracting Officer for=20 approval, in writing, 72 hours in advance of the scheduled change. 1.5.2. Holidays: The contractor shall provide emergency support, if needed, only on the following=20 holiday: New Year?s Day Martin Luther King Jr. Birthday Presidents Day Memorial Day Independence Day Labor Day Columbus Day Veterans Day Thanksgiving Day Christmas Day Inauguration Day 1.6 Emergency Service: The contractor shall provide emergency service calls at any=20 time and shall respond within (4) hours of notification. The contractor shall furnish=20 telephone number for emergency calls. Calls received by answering service or=20 machine will be considered as notification. 1.7. Conservation of Utilities: The contractor shall make sure employees practice utility=20 conservation. The contractor shall be responsible for operating under conditions that=20 prevent the waste of utilities to include: 1.7.1. Lights shall be used only in areas where work is actually being performed. 1.7.2. Employees shall not adjust mechanical equipment controls for heating, ventilation,=20 and air conditioning systems. 1.7.3. Water faucets or valves shall be turned off when not in use. SECTION C-2 SPECIFIC TASKS 2.1 Scope of Work: Contractor shall furnish all labor and equipment to provide full=20 inspection, maintenance, and repair for the equipment listed in this Statement of=20 Work. Work shall be performed in accordance with the terms as stated herein. 2.1.1. The contractor shall make inspections in December, March, and September. The=20 contractor shall perform all required functions, repairs, and services during these=20 inspections. Length of time for inspection will be one week (40 hours). 2.1.2. Coordination of Work Operation: The work to be performed under this contract=20 is on an active facility, therefore the contractor, prior to starting any work under=20 this contract, shall coordinate his work operations and inspections with the Quality=20 Assurance Evaluator and the Plant Foreman. 2.3. Exclusives: It is understood that the service and maintenance provided for herein=20 does not include the following: =20 a. Normal daily and weekend functions of stopping and starting the equipment. b. Repairs to equipment caused by freezing, corrosion, erosion, lightning, electrolytic action or any other reason beyond the contractor?s control. c. Damage caused by fire, vandalism, war, sabotage, or insurrection. d. Damage caused by improper operation or unauthorized repair. 2.4. Preventive Maintenance: The following preventive maintenance and calibration=20 shall be performed on the listed equipment during each scheduled visit: a. Check Flame Safeguard system on the three (3) 25 million BTU, HTHW Generators. (1) Test all limits for both gas and oil firing (2) Test FSP Enunciator (3) Check all interlocks for start up and shut down cycles, including but not limited to Purge, Low Fire Start, Fuel Valve Closing, Pilot Ignition and Flame Scanner. Compare data to original test data; adjust as required to maintain original data. b. Check combustion of both gas and oil to six- (6) load points (minimum, 20%, 40%, 60%, 80%, and 100% firing rate. Compare data to original test data, adjusting fuel to air ratios as required to keep unit efficiency to original set levels c. On all equipment listed in Section C-4.1.1. 2.4.1. The contractor shall provide a checklist for each system inspection and shall submit=20 a copy to the Contracting Officer for Government approval before the first=20 inspection. The checklist shall be used during each system inspection and shall be=20 signed by the contractor after task is completed. A copy of the completed report=20 will be submitted to the Quality Assurance Evaluator. 2.4.2. A completed report of all tests will be submitted to the Service Contracting Office within five (5) working days after each inspection/service visit. Reports will include results and records of condition found, adjustment, calibration sheet repairs or replacement made and any pertinent recommendation for major tear down, repair, overhaul, rebuild, and/or replacement of components. 2.4.3. The contractor shall review operating procedures on IBM Troll talk-PC Software and hardware with plant foreman and daytime personnel. Contractor shall perform required maintenance, diagnostic assistance of system software when unexplained shutdown is experienced and make minor program changes in accordance with Air Force Policy or recommended changes by the plant manager. 2.5. Interruption of Utilities: The contractor shall not interrupt existing utilities on the base without first obtaining prior written permission from the Quality Assurance Evaluator (QAE), a minimum of seven (7) work days in advance. The request, in writing, for any desired outage, shall state the date and duration of proposed outages. The contractor shall make all final connections to existing utility services, furnish all material, and perform all excavation, backfilling, ramping, and all other labor incidental to making the final connection.=20 2.6. Clean-Up All material and scrap remaining from new work and classified as trash/debris shall be removed from the Base and disposed of by the contractor in a legally acceptable manner. It shall be the responsibility of the contractor to assure proper loading of all vehicles used for transport of materials and debris. The contractor at no additional cost to the government shall accomplish all clean-up activities. SECTION C-3 CONTRACTOR FURNISHRD ITEMS 3.1 MATERIALS, PARTS, AND SUPPLIES ACQUISITIONS: 3.1.1. The contractor shall acquire the materials, parts, and supplies required for the performance of this contract and shall be responsible for the cost of all materials, parts and supplies less than $500.00. In the event of a justifiable contractor declared individual item expense over $500.00, the contractor is to contact the QAE at 767-1257 for approval of purchase. The QAE in turn will contact the Contracting Officer. All replacement parts over $500.00 obtained by the contractor will be separately invoiced. A copy of each approved order from suppliers? invoice and other necessary supporting documents shall be attached to the invoice. All purchase documents shall be made available for the Contracting Officer?s review and audit. The government reserves the right to specify quality, size, efficiency and aesthetic requirements of reimbursable parts. 3.1.2. The billing for reimbursable parts shall be supported by an itemized list of all reimbursable materials used showing the items, the number, unit cost, and total cost. The contractor shall maintain original delivery tickets, sales slips or other documents identifying items purchased under the ? reimbursable parts? portion of this contract. All purchase documents shall be available for the Contracting Officer?s review and audit. 3.1.3. QUALITY. All replacement parts for the control system shall be the same as the manufacturer?s installed. Where components are no longer produced, the system shall be revised or upgraded for new components. The contractor shall submit a proposal for all such revisions to the contracting officer for approval prior to starting the work. 3.1.4. WARRANTY. The contractor shall be responsible for replacing any warranty/guarantee items within appropriate period of such warranty/guarantee. This shall include manufacturer warranty/guarantees. 3.1.5. EQUIPMENT. The contractor shall have in his possession the following minimum equipment that is needed to completely service, calibrate, maintain, troubleshoot, and define problems and effect solutions at the plant site. These tools must consist of but limited to the following: a. Field service oscilloscope b. Pulse generator c. Pulse counter d. Two (2) digital multi meters e. Low voltage instrument signal generator f. Milliampere instrument signal generator g. Ultrasonic demagnetizer h. Portable computer for flow calculation comparison and verification i. Portable combustion analyzer for boiler flue gas tests to include: (1) Excess Oxygen (2) Percent Combustibles (3) Stack Flue Gas Temperature Rise (4) Calculated Percent Combustible Efficiency for each Specific Fuel j. Full set of instruction books with parts lists, operational maintenance and troubleshoot guides for each piece of equipment included in the service contract. k. Hand tools for instrument, flue gas analyzers, flame safety systems, boiler controls and burners. l. Bristol Babcock hand held configurator used on Model #3711-40A Trace Recorder SECTION C-4 GOVERNMENT OWNED EQUIPMENT 4.1. General. The government shall operate this equipment in accordance with=20 manufacture?s recommended instructions and promptly notify the contractor of any=20 unusual operating conditions. The government shall provide access to the equipment=20 including removal
