Equipment Maintenance at Elmendorf AFB Gym -This is a combined synopsis/solicitation for commercial items prepared in accordance with FAR Subpart 12.6. PLEASE READ THIS ENTIRE NOTICE CAREFULLY AS IT CONSTITUTES THE ONLY NOTICE THAT WILL BE ISSUED. This is a request for quotation (RFQ). The incorporated document provisions and clauses are those in effect through Federal Acquisition Circular 97-27. The Standard Industrial Classification (SIC) Code is 5941. The NAICS Code is 811310. PROJECT:=20 1. SCOPE OF WORK:=20 a) Repair broken equipment. b)Do preventative maintenance on equipment: Strength machines:Inspect cables,bands,bolts,and handgrips. Ensure guide rods are lubricated weekly and are free of buildup. Internal preventative maintenance. Bikes:Each month or when needed,you should check the handrails,pedals,and shroud are secure. Remove shroud and vacuum interior;avoid bumping or damaging parts normally covered by shroud. Steppers/Step Mills: Clean the step chain assemblies with mild degreaser then relubricate with 30W motor oil. Clean the bearing plate with mild degreaser then re-lubricate with multi-purpose grease. Lubricate the step hinges with 30W motor oil or light bearing grease. Lubricate the drive chain with 30W motor oil. Lubricate the step chain assemblies with 30W motor oil.Inspect the timing belt for any damage. Adjust if necessary.Inspect the frame for rust,chipping paint or bubbling.Touch up as needed. Treadmills:Inspect the running belt for alignment and tension. Ensure the power cord is not under the treadmill and away from the elevation screw. Inspect the inside surfaceof the running belt and the top surface of the running deck for unusual wear. Check whether the belt feels glazed (a smooth almost melted appearance).Inspect the display panel/keypad for wear. Remove motor shroud and suspend it from the display neck with a Bungie Cord. Vacuum around the motor and its ventilation holes;avoid bumping or damaging parts normally covered by the shroud. To ensure the longevityof the running belt with a soft,dry cloth. To clean,slide the cloth between the running belt and deck from one side of the frame to the other. Then hold the edges of the cloth,pull the cloth fro, the head-roller down to the tail roller. Wipe the elevation screws down with a clean dry cloth and coat them with silicon lubricant spray to prevent rust accumulation.Re-wax running deck every 2,000 miles. Cross Trainers:Inspect arms to make sure they are not loose.Inspect and clean main back of machine etc. Spinning Bikes: Inspect bikes for loose parts weekly, especially pedals.Perform preventative maintenance on spinning bikes. c) Recover old and torn strength machine pads. d) On Call: Monday through Friday,0730-1630. e) Regularly scheduled preventative maintenance will be each Thursday. f)In the event critical equipment breaks, a 24 hour response time is necessary to fix the equipment or identify the problem and order necessary replacement parts. Critical equipment includes treadmills and ellipticals. g) List of equipment that is to be worked on: Cardio Equipment: 13 Treadmills,9 upright bikes,12 recumbent bikes,12 steppers/step mills,8 cross trainers,25 spinning bikes. Strength Machines:2 leg presses,1 chest press,1 overhead press,3 lat pull down,1 compound row,1 leg extension,1 leg curl,2 pec dec fly,1 arm curl,1 tricep press,1 ab crunch,1 back extension,1 rotary calf,1 cable crossover,1 high/low pull,1 leg abduction,2 asssisted dip/chin,1 smith machine. Delivery terms shall be FOB Destination, Elmendorf AFB, Alaska 99506. The following provisions and clauses are incorporated and shall remain in full force in any resultant purchase order; FAR Part 52.212-1 Instructions to Offerors-Commercial Items, is incorporated by reference. The provision at 52.212-2 Evaluation of Commercial Items, applies to this solicitation. Award will be based on low price/technically acceptable offeror. The Government reserves the right to make award on the initial quote received without discussions. Each offeror shall include a completed copy of the provision at FAR 52.212-3 Offeror Representations and Certifications-Commercial Items with its quote. FAR Part 52.212-4 Contract Terms and Conditions-Commercial Items, is incorporated by reference and applies to this acquisition. Note the following addenda to FAR 52.212-4: Incorporate DFARS 252.204-7004 Required Central Contractor Registration. Incorporate the clause by reference at 52.212-5 Contract Terms and Conditions Required to Implement Statutes or Executive Orders-Commercial Items. However, for paragraph (b) only the following clauses apply: 52.222-21 Prohibition of Segregated Facilities, 52.222-26 Equal Opportunity, 52.222-35 Affirmative Action for Disabled Veterans and Veterans of the Vietnam Era, 52.222-36 Affirmative Action for Handicapped Workers with Disabilities, 52.222-37 Employment Reports on Disabled Veterans and Veterans of the Vietnam Era, and 52.232-33 Payment by Electronic Funds Transfer-Central Contractor Registration. For paragraph (c) the following clauses apply: 52.222-41 Service Contract Act of 1965, As amended, 52.222-42 Statement of Equivalent Rates for Federal Hires, and 52.222-47 SCA Minimum Wages and Fringe Benefits Applicable to Successor Contract Pursuant to Predecessor Contractor Collective Bargaining Agreement (CBA). FAR 52.212-5 Contract Terms and Conditions Required to Implement Statutes or Executive Order?Commercial Items (XXX 2001) (DEVIATION). DFARS 252.222-7000 Restrictions on Employment of Personnel (Mar 2000)(IAW DFARS 222.7003). Wage Determination No. 94-2017, Revision 25 dated 8/23/2001, hereby applies. This is a DO rated order with priority identification symbol of S10. A Firm-Fixed Price purchase order will be issued under the Simplified Acquisitions Procedures. Submit signed and dated offer to 3CONS/LGCB at or before 18 October 2001 4:00PM Alaska Standard Time. Quotation shall be signed, dated and submitted on your company?s letterhead or quotation forms.
