Description
This is a combined synopsis/solicitation for commercial items prepared in accordance with the format in far subpart 12.6,as supplemented with additional information included in this notice for the South Texas Veterans Health Care System. This announcement constitutes the only solicitation; Request for Quote (RFQ), a written solicitation will not be issued. Solicitation documents and incorporated provisions and clauses are those in effect through federal acquisition circular 97-27. NAICS 811219 with business size standard $5M. Prospective vendors are required to submit written proposal for full service on-site service/maintenance agreement for government owned Ventilators and Anesthesia Apparatus, manufactured by Datex-Ohmeda. The Government anticipates to award a single firm-fixed price contract to the contractor providing the best value for the Government in regards to technical expertise, past performance and price. Technical expertise and past performance when combined are significantly more important than cost. The following items and quantities will apply Contractor will perform quarterly(4ea) preventive maintenance inspections, in addition to unlimited emergency calls for the following equipment: ventilators (part of anesthesia machine) Model 7800 (3ea) model 7810 (1ea), Anesthesia machine model Excel (3ea), Anesthesia machine model Mod II plus with GMS (1ea), anesthesia machine model CD (8ea), anesthesia vaporizers(part of 3 previous types anesthesia machines) (12ea), Oximeter model 3710(1ea), C02 monitor model 5210 (1ea), RGMS model 5250 (3ea). Services to include parts, labor, equipment, tools, travel and insurance for the period of date of award to September 30, 2002, with expected award date NLT November 1, 2001. Routine maintenance hours will be from 7:00 to 5:30pm Monday thru Friday. All maintenance inspections will be scheduled and coordinated with facility. Emergency response calls will be no more than four (4) hours with (2) hour minimum telephone response call back. All service technicians must have certification for service and/or repair from manufacturer Datex-Ohmeda. Alterations made to system will be made only with the consent of the Government. ALL OFFERORS ARE TO INCLUDE A COMPLETED COPY OF THE PROVISIONS AT 52.212-3, OFFEROR REPRESENTATIONS AND CERTIFICATIONS COMMERCIAL ITEMS, WITH OFFER.
