Description

This is a combined synopsis/solicitation for commercial items prepared in accordance with the format in Subpart 12.6, as supplemented with additional information included in this notice. This announcement constitutes the only solicitation. Proposals are being requested and a written solicitation will not be issused. Reference #F7SPT2122601 is issued as a request for quotation (RFQ). The solicitation document, incorporated provisions and clauses are those in effect through the Federal Acquisition Circular 97-13. This requirement is set-aside 100% for small business. The NAIC code 713940, Physical Fitness facilities, and small business size standard is $5 million. Please provide quote for the monthly service fee and approximately 75 hours of emergency calls and reimbursable replacement parts in accordance with the Statement of work (see attachment) Interested parties capable of providing the above, must submit written price and technical proposals, and provide information on their past performance to include: contract number, references (names), and phone numbers. All responses will be fully considered. Award will be made to the responsible offeror whose offer conforms to this requirement and is considered technically acceptable.
