- Description
- This is a combined synopsis/solicitation for commercial items prepared in accordance with the format in Federal Acquisition Regulation (FAR) Subpart 12.6, as supplemented with additional information included in this notice. The announcement constitutes the only solicitation; quotations are being requested and a written solicitation will not be issued. This solicitation is being issued as a Request for Quotation (RFQ)453-0042, the provisions and clauses incorporated are those in effect through Federal Acquisition Circular (FAC) 97-27. This solicitation is under the Simplified Acquisition Procedures (SAP). This procurement is unrestricted. The North America Industry Classification System (NAICS) Code is 421450, with a small business size standard of 100 employees. The Federal Bureau of Prisons, Federal Medical Center, Butner, North Carolina, has a requirement for the following Brand Name or Equal Equipment: Brand Name-PMX Medical; A.E.D. Lifesaver System; qty. 15 each. A.E.D. Lifesaver System including -- Single compact Cordura Nylon wall-hanging case with 3-sided angle zipper permitting immediate access and visibility of internal equipment and supplies which include: Phillips/Agilent Technologies biaphasic automatic cardiac defibrillator with easy to use voice prompts; long-life battery technology; automatic self-testing. MediTech BNOS Rescue Vent, time cycled automatic ventilator and CPR timer; Rescue Vent providing rescue breaths of 100% oxygen to sudden cardiac arrest victim. Flotec Lifesaver Regulator with user adjustable settings: CPR & Flow, capable of driving Rescue Vent to 250PSI. Aluminum C or M9 sized compact oxygen cylinder with "no tools" toggle valve, capable of holding 245 liters of oxygen (15-20 minutes). System includes resuscitation face mask, capable of making secure seal to Rescue Vent, and a Hudson Non-Breather mask for oxygen administration at 12LPM to a responsive adult in chest pain. Standard Commercial Warranty. FOB-Destination Butner, North Carolina. -- The above characteristics are intended to be descriptive, but not restrictive. Unless clearly marked that the offeror is quoting on an "equal" the offered item shall be considered the brand name product referenced in this announcement. If the vendor proposes to furnish an "equal" product, then the brand name of the "equal"product, if any, shall be indicated on the Request for Quotation (RFQ). The evaluation and the determination as to the equality of the product, if not the described brand name, shall be the responsibility of the Government and will be based on information furnished by the offeror. The Government's purchasing activity is not responsible for locating or securing any information which is not identified in the quotation and reasonably available. To ensure sufficient information is available, the offeror must furnish as a part of the quotation- all descriptive material (such as cuts, illustrations, drawings or other information) necessary for the Government to determine whether the "equal" product meets the salient characteristics of the requirement. If the offeror proposes to modify a product so as to make it conform to the requirement, then proposed modifications shall clearly be marked with descriptive materials to show modifications. The government reserves the right to request product samples which can be tested for compatibility and provided at no additional cost. FAR 52.211-6 Brand Name or Equal (Aug. 1999); FAR provision 52.212-2 Evaluation -Commercial Items (Jan. 1999); Justice Acquisition Regulation Clause 2852.211-70 Brand-Name or Equal (Jan. 1985) Each offeror shall include a completed copy of the FAR Provisions at 52.212-3, Offeror Representations and Certifications (May 2001) with its offer. The Government will award to the responsible offeror whose offer conforming to this solicitation will be most advantageous to the Government, price and other factors considered.
