Elmendorf Air Force Base Aerobics Instruction-This is a combined synopsis/solicitation for commercial items prepared in accordance with FAR Subpart 12.6. PLEASE READ THIS ENTIRE NOTICE CAREFULLY AS IT CONSTITUTES THE ONLY NOTICE THAT WILL BE ISSUED. This is a request for quotation (RFQ). The incorporated document provisions and clauses are those in effect through Federal Acquisition Circular 97-12. The Standard Industrial Classification (SIC) Code is 7999. The NAICS Code is 713940. PROJECT:=20 1. SCOPE OF WORK: Contractor shall conduct and administer all aspects of the exercise portion of the Weight Management and Fitness Enhancement Programs held in the Base Fitness Center at Elmendorf Air Force Base, Building 9510. 2. PERSONNEL: The contractor or designated representative shall meet with government personnel designated by the Contracting office to discuss immediate problems. The Contractor or representative shall respond within four hours after notification. The contractor shall provide the telephone number of the person(s) to call should the need arise. 3. EMPLOYEE IDENTIFICATION: Contractor personnel must be recognizable as such while in the Fitness Center. This should be accomplished by wearing distinctive athletic attire. The contractor is responsible for acquiring and maintaining an appropriate number of uniforms to meet their need at their expense. 4. SPECIFIC DUTIES: Position is to manage, supervise, and conduct the Elmendorf Air Force Base Weight Management and Fitness Enhancement Program. Contractor will be responsible for the following: a. Individual must provide to the Contracting Office personnel documents certifying accreditation in physical conditioning, weight management, or certification from other recognizable institutes. As a minimum the individual must be certified by a nationally accredited organization as an aerobic instructor(i.e. ACE, Dante). Classes will consist of an aerobic type workout. Classes should be well-rounded and include, but not limited to: step aerobics, slide aerobics, high impact, low impact, modified, water aerobics and pre-natal instruction. NOTE: At least one class taught during the day must have a person trained to give advise and instruction in pre-natal programs. The contractor will submit a monthly schedule to the Athletic Director for approval. The schedule will include the instructors name, type of class, and time. b. Must keep current and provide proof of certification in Cardiopulmonary Resuscitation as outlined by the American Medical Association or the American Red Cross. c. Contractor will perform classes using the following schedule: Aerobics Oct -- Mar classes at 0600, 1130, 1715, and 1830 hrs Monday ? Friday, Saturday at 1130 Apr -- Sep classes at 0600, 1130, and 1730 hrs Monday ? Friday, Saturday at 1130 Spinning ? Mon, Wed, Fri at 1100 and 1730, Sat 1100 Water Aerobics ? Mon, Wed, & Fri at 1800 These times are subject to change based on the needs of the active units. Contractors should base their quotation on providing no more than 1,455 classes during the course of a year. Price shall be quoted on a per class basis. Contractor will be notified 30 days in advance of any changes in class times. The program is outlined in AFI 34-107 Section B, paragraph 10. d. Contractor shall provide all tapes and music requirements for the classes. e. Contractor will ensure that all equipment (steps, mats, bands, music equipment etc.) is used according to the Fitness Center?s policies and are properly put away by the students or instructor after class. f. The contractor will handout at least 10 customer comment cards per day as provides by the fitness center. g. The contractor shall be financially responsible for any damage caused to fitness center equipment while it is under their control ( i.e. stereo system, microphone/headset, ect.) to include repair/replace, maintenance, shipping and handling of damaged equipment h. The contractor will record an accurate headcount for each class performed using forms provided by the fitness center. i. The contractor will perform a check of students heart rate at least three times per class. j. The contractor will limit the number of classes taught by one instructor to no more than two classes per day. 5. The following holidays will be recognized and no instruction will be necessary: Thanksgiving Day, and Christmas Day. For other Federal Holidays and Down/Family Days only one aerobic class, 1130, and one spinning class will be taught. Classes will be taught on Good Friday. Delivery terms shall be FOB Destination, Elmendorf AFB, Alaska 99506. The following provisions and clauses are incorporated and shall remain in full force in any resultant purchase order; FAR Part 52.212-1 Instructions to Offerors-Commercial Items, is incorporated by reference. The provision at 52.212-2 Evaluation of Commercial Items, applies to this solicitation. Award will be based on low price/technically acceptable offeror. The Government reserves the right to make award on the initial quote received without discussions. Each offeror shall include a completed copy of the provision at FAR 52.212-3 Offeror Representations and Certifications-Commercial Items with its quote. FAR Part 52.212-4 Contract Terms and Conditions-Commercial Items, is incorporated by reference and applies to this acquisition. Note the following addenda to FAR 52.212-4: Incorporate DFARS 252.204-7004 Required Central Contractor Registration. Incorporate the clause by reference at 52.212-5 Contract Terms and Conditions Required to Implement Statutes or Executive Orders-Commercial Items. However, for paragraph (b) only the following clauses apply: 52.222-21 Prohibition of Segregated Facilities, 52.222-26 Equal Opportunity, 52.222-35 Affirmative Action for Disabled Veterans and Veterans of the Vietnam Era, 52.222-36 Affirmative Action for Handicapped Workers with Disabilities, 52.222-37 Employment Reports on Disabled Veterans and Veterans of the Vietnam Era, and 52.232-33 Payment by Electronic Funds Transfer-Central Contractor Registration. For paragraph (c) the following clauses apply: 52.222-41 Service Contract Act of 1965, As amended, 52.222-42 Statement of Equivalent Rates for Federal Hires, and 52.222-47 SCA Minimum Wages and Fringe Benefits Applicable to Successor Contract Pursuant to Predecessor Contractor Collective Bargaining Agreement (CBA). FAR 52.212-5 Contract Terms and Conditions Required to Implement Statutes or Executive Order?Commercial Items (XXX 2001) (DEVIATION). DFARS 252.222-7000 Restrictions on Employment of Personnel (Mar 2000)(IAW DFARS 222.7003). Wage Determination No. 94-2017, Revision 25 dated 8/23/2001, hereby applies. This is a DO rated order with priority identification symbol of S10. A Firm-Fixed Price purchase order will be issued under the Simplified Acquisitions Procedures. Submit signed and dated offer to 3CONS/LGCB at or before 26 September 2001 4:00PM Alaska Standard Time. Quotation shall be signed, dated and submitted on your company?s letterhead or quotation forms.
