Description: (i) This is a combined synopsis/solicitation for commercial items prepared in accordance with the format Subpart 12.6, as supplemented with additional information included in this notice. This announcement constitutes the only solicitation; proposals are being requested and a written solicitation will not be issued. (ii) Request for Quotation, F5HOSP11830300. (iii) The solicitation document and incorporated provisions and clauses are those in effect through Federal Acquisition Circular 97-27. (iv) Notice: This solicitation is full and open competition. The set-aside for small business has been dissolved. The North American Industry Classification is 334517. The size standard is 500. (v) Provide a hardware and software, corrective and preventive maintenance program to include a complete set of special service tools, test equipment, fixtures, spare parts, verification procedures and schedules to provide an operational check of the system concurrent with normal operation and with no degradation in system performance, software problem analysis, associated reprogramming and corrected software documentation. ITEM 0001: CT Highlight Advantage less glassware, QTY: 1EA. ITEM 0002: Glassware (76,000-100,000 slices per year), QTY: 1EA ITEM 0003: GE Radiographic Room less glassware, QTY: 1EA ITEM 0004: GE R&F Room less glassware, QTY: 1EA ITEM 0005: GE R&F Specials Room less glassware, QTY: 1EA (vi) Additional description can be found in attached performance work statement.
