AMENDMENT 1 ? 16 SEP 01 The following changes have been made to subject solicitation. a. The following has been added to line item 0001: Custom Designed ROTO case to securely fit CCTV Cameras. Camera must be housed inside case as to prevent damage when being transported in tactical environment.=20 b. The response date has been changed from 21 Sep 01, to now read 26 Sep 01. c. All other terms and conditions of this combined synopsis / solicitation remain unchanged. =20 This is a combined synopsis/solicitation for commercial items prepared in accordance with the format in Subpart 12.6, as supplemented with additional information included in this notice. This announcement constitutes the only solicitation; proposals are being requested and a written solicitation will not be issued. The solicitation number is F1A502122004 and is issued as a request for proposal (RFP). The solicitation document and incorporated provisions and clauses are those in effect through Federal Acquisition Circular 97-26. This notice is 100% set-aside for small business concerns. The associated NAICS code is 421410 and small business size standard is 100. Request proposal on the following item and quantity: 0001 ? 8(ea) CCTV System to include the following items in each system: 8(ea) Lorex 1/3? CCD color submersible camera, 1(ea) Lorex Multiview 9 camer switcher, 2(ea)Phillips 9? color monitor, 1(ea) Pelican 1650 Case(29?x 18?x 10?), 1(ea) Rotocase(28?x 22?x 18?) Custom Designed ROTO case to securely fit CCTV Cameras. Camera must be housed inside case as to prevent damage when being transported in tactical environment. Items are to be delivered FOB destination to HQ AFSOC/INS/DIS, 100 Bartley Street, Suite 105 West, Hurlburt Field, FL 32544 for acceptance by Mr. Brian Butts. Offerors shall provide a brand name or equal on all components contained in each CCTV system. Offerors providing an equal product must provide specification sheets or product literature for evaluation by requesting activity with their proposal submission. The provision at 52.212-1, Instructions to Offerors -- Commercial, applies to this acquisition and includes the following addenda: (1) Add the following paragraph: (b)(12) Each offeror shall submit their signed and dated offer (signed by an official authorized to bind your organization in a contract) in one original and one copy to 16th Contracting Squadron, Attn: Aaron Kruse, P.O. Box 9190, 350 Tully Street, Hurlburt Field FL 32544. Proposals are due on 21 Sep 2001 at 4:00PM Local time and must reference the proposal number on the outside of your package for identification purposes. Faxed or e-mailed proposals will not be accepted. The provision at 52.212-2, Evaluation -- Commercial Items, applies to this acquisition. The significant evaluation factors will be based on price or price related factors. Proposals shall include a completed copy of the provision at 52.212-3, Offeror Representations and Certifications -- Commercial Items, with its offer. Failure to do so may result in your proposal to be considered non-responsive. The clause at 52.212-4, Contract Terms and Conditions -- Commercial Items, and 52.212-5, Contract Terms and Conditions Required To Implement Statutes Or Executive Orders -- Commercial Items, applies to this acquisition. Defense FAR Supplement (DFARS) provisions and clauses apply to this solicitation and are incorporated by reference: (1) DFARS 252.212-7001, Contract Terms and conditions required to Implement Statutes or Executive Orders Applicable to Defense Acquisition of Commercial Items. The following clauses under paragraph (b) of 252.212-7001 are amended to read: 252.225-7001, Buy American Act and Balance of Payment Program, 252.225-7007, Buy American Act-Trade Agreements-Balance of Payment Program, 252.225-7036, Bur American Act-North American Free Trade Agreement Implementation Act-Balance of Payment Program.
