- SOLICITATION NO. 550-24-01 IS HEREBY AMENDED IN ITS ENTIRETY. This is a combined synopsis/solicitation for commercial items prepared in accordance with the format in Subpart 12.5, as supplemented with additional information included in this notice. This announcement constitutes the only solicitation; proposals are being requested, and a written solicitation will not be issued. This procurement is being conducted under Simplified Acquisition Procedures in accordance with FAR Part 13 and FAR Part 12, Acquisition of Commercial Items. This combined synopsis/solicitation is being issued as an Invitation For Bid (IFB) 550-24-01. The solicitation document and incorporated provisions and clauses are those in effect through Federal Acquisition Circular (FAC) 97-27. IFB 550-24-01 shall be referenced on all proposals submitted. This is a unrestricted procurement. The NAICS Code is 334516 and the small business size is 500 employees. The VA intends to solicit proposals and award a firm-fixed price, estimated reagent quantities contract. The successful contractor shall provide delivery, FOB: Destination VA Illiana Health Care System, 1900 East Main Street, Danville, IL 61832-5198 as follows: ITEM NO. 1: AUTOMATED CLINICAL CHEMISTRY ANALYZER, BENCHTOP, FOR THE ANALYSIS OF SERUM, PLASMA, AND URINE. Salient characteristics include but are not limited to: a.) Dimensions of 52 inches wide x 30 inches high x 26 inches deep; b.) No preparation of reagents; c.) No pretreatment of HDL and Hbalc assays; d.) 36 on board test capacity; e.) Menu of 130 different test applications; f.) Auto-dilution of urine samples; g.) 90 samples on board; h.) Uses Windows NT software; i.) Uses less than 5 liters of water per hour; j.) Uses absorbence photometry & fluorescence Polarization Technology; k.) Able to run Lithium Tests; 1.) Capability of providing Hbalc at the time of outpatient visit as mandated by Veterans Health Administration (VHA) Directive 99-042; m.) Analyzer must be capable of doing random access Hbalc, one sample at a time, during veteran's outpatient visit; n.) Capability of analyzer to provide standardized LDL analysis results between VA IHCS and other medical centers within VISN 11. 0.) Includes Millipore Water System Catalog No. DH20400B or equal; and p.) Instrument Cart Catalog No. 8152853 for Millipore Water System or equal (ROCE COBAS INTEGRA 400 ANALYZER or equal) /Quantity: l each @ $_________. ITEM NO. 2: FIVE-YEAR REAGENT DISCOUNT TO BE DEDUCTED FROM THE PURCHASE PRICE (COST) OF ITEM NO. 1 Quantity: l each @ $_______ DISCOUNT. ITEM NO. 3: INSTALLATION START-UP KIT (Contractor shall submit a complete listing of all reagents, calibrators, etc. byname, stock number, and quantity with his/her proposal. Contractor shall be responsible for providing all chemicals, reagents, calibrators, supplies, etc. used during the equipment installation process by company's installer to check calibration, precision, and accuracy of equipment at no charge to VA Quantity: 1 lot @ $________/LOT. ITEM NO. 4: ONE-YEAR WARRANTY TO INCLUDE ONE YEAR SERVICE AND MAINTENANCE WHICH INCLUDES FOUR (4) annual preventative maintenance visits; unlimited on-site emergency service; or site response within 24 hours; and a 24-hour technical telephone support Quantity: 1 each @ $________. ITEM NO. 5: INSTALLATION (Contractor shall submit a proposal outlining his/her responsibilities and installation procedures) ..Quantity: 1 job @ $_______ PER JOB. ITEM NO. 6: OFF-SITE TRAINING FOR TWO OPERATORS with to include all transportation, meals, and lodging as well as additional on-site training as needed. ITEM NO. 9: CONTRACTOR SHALL PROVIDE A SEPARATE FIVE-YEAR PRICING SCHEDULE listing reagent name, catalog number, tests per kit, price per kit, and number of kits per year. Note to Offeror: Quantities listed for Item No. 9 are estimated quantities only. No price adjustment shall be made should VA not be able to meet the quantities estimated. ITEM NO. 10: CONTRACTOR SHALL PROVIDE A SEPARATE FIVE-YEAR PRICING SCHEDULE listing all calibrators, controls, and other supplies by name, catalog number, price per unit, and number of kits required per year to accomplish the estimated volume of reagents, tests per year listed in Item No. 9 . ITEM NO. 11: Contractor shall also provide an IMPLEMENTATION PLAN AND DELIVERY SCHEDULE WITH HIS/HER OFFER listing company contacts, phone and fax numbers, and 24-hour technical support phone number. The contract is subject to the availability of VA appropriations. In accordance with The Health Insurance Portability and Accountability Act (HIPAA) and the Balanced Budget Act (BBA) of 1977, the Office of Inspector General has established a list of parties and entities excluded from Federal health care programs, Specifically, the listed parties and entities may not receive Federal Health Care program payments due to fraud and/or abuse of the Medicare and Medicaid programs. Therefore, all offerors shall review the OIG List of Excluded Individuals/Entities on OIG web site at www.hhs.gov/oig to ensure that the proposed candidate(s) and/or firm(s) are not listed. Offerors should note that any excluded individual or entity that submits a claim for reimbursement to a Federal health care program, or causes such a claim to be submitted, may be subject to a Civil Monetary Penalty (CMP) of $10,000 for each item or service furnished during a period that the person or entity was excluded and may also be subject to treble damages for the amount claimed for each item or service.
