Description

Provide Community Based Outpatient Care (CBOC) Primary Medical Care Service to beneficiaries of the Department of Veterans Affairs. Services are to be provided to approximately 200 eligible veterans at contractors clinics located in the Faribault Area of Southern Minnesota. This contract is intended for twelve (12) months beginning on or about October 1, 2001, with two (2) additional twelve (12) month renewal options.
