Description
This Is a combined synopsis/solicitation notice of intent to obtain a full service maintenance agreement prepared in accordance with the format in FAR Subpart 12, as supplemented with additional information included in this notice. This announcement constitutes the only solicitation; proposals are not being requested and a written solicitation will not be issued. The solicitation number for this acquisition is 674-02-10 and the solicitation is being issued as a notice of intent. This solicitation document and its incorporated provisions are those in effect through Federal Acquisition Circular Fac 97-23. The Central Texas Veterans Health Care System intends to negotiate on a sole-source basis using simplified acquisition procedures with Eastman Kodak/Health Science Division of Dallas, TX to provide First Response Service agreement including training and manuals for Kodak Dry View Processors and PACs System Links located at Temple, Waco, Marlin and Austin, TX listed as follows in the quantities indicated: Dry View Laser Imager M#8700P, K#18706768,Serial #8706768, one (1) Year (yr); Dry View Laser Imager M#8700P, K#3855614, Serial #8705188, one (1) yr; Dry View Laser Imager, M#8300S, K#41643661, Serial #CA81519, one (1) yr; Dry View Laser Imager, m#8700N, K#41640944, Serial #8708430, one (1) yr; PACs Link Printer Mod Server M#9401C, K#41640942, Serial #101918, one (1) yr; PACs Link Modality Server, M#9410A, K#41640943, Serial #101919, one (1) yr; Dry View Laser Imager, M#8700N, K#41643732, Serial #8708103, one (1) yr; PACs Link Prt/Mod Server, M#9410C, K#41643733, Serial #103026, one (1) yr; Training for Biomedical Technician/s as part of the First Response Agreement on Model #'s 8300, one (1) each; and, Training for Biomedical Technicians as part of the First Response Agreement on Model #8700, one (1) each. This is not a Small Business Set Aside. The North American Industry Classification System (NAICS) Code for this service is 811213 and the small-business size standard is 500. This is a fixed-price contract and the FOB terms are "Destination". The anticipated period of performance will be 10/1/01 9/30/02 and two (2) option years. It is essential that the same equipment continue to be used in order to maintain data integrity. Changing equipment would interfere with agency mission of patient care. FAR 52.225-3 Buy American Act also applies. Offerors must include with their offer a completed copy of the provisions Commercial Items. Offers must be submitted no later than 4:30 P.M.
