Description
Amendment "A" to IFB 658-60-01 is hereby issued, and provides for the following: Faxed bids will be accepted and should be faxed to the attention of the Contracting Officer (90C3) at (540) 983-1065. Faxed bids must be received by 2:00 p.m. (local time), Wednesday, September 19, 2001, with original documents to follow via U.S. Mail, other carrier, or handcarried. The Government is not responsible for illegible faxed material. Receipt of Amendment "A" must be acknowledged as part of the Contractors bid package. During the pre-bid conference/site visit held on Wednesday, September 12, 2001, the following questions were raised. QUESTION 1) Should Contractors reference sample numbers from prior surveys in their reports? ANSWER: Yes, whenever applicable, especially information from 1996 survey for Building 12, which will be made available to Contractor receiving, award. QUESTION 2) Will plaster samples be required? ANSWER: Yes, at least two (2) per building in accordance with Attachment A. QUESTION 3) What kind of access is there to the water towers? ANSWER: The Salem, VAMC has no lift trucks available to be used for access to the towers. They both have ladders. Contractor shall be required to follow all Federal, State, and local safety requirements in gaining access to the towers. QUESTION 4) Can all surveys be done during normal business hours or will some surveys need to be done after hours? ANSWER: Most of the areas listed on Attachment "A" are not in direct patient care areas and can be done during normal business hours. Normal business hours are 8:00 a.m. 4:30 p.m. If required, arrangements can be made for the Contractor to work 10 hour days. QUESTION 5) What type of lead survey is required? ANSWER: Paint chip tests will be required in areas as indicated by XRF testing. QUESTION 6) Will floor plans be available? ANSWER: Computer floor plans will be made available to Contractor receiving award. QUESTION 7) What is the time frame for completion of survey? ANSWER: Survey should be completed within 120 calendar days after contract award. QUESTION 8) Will space be made available to Contractor to use during survey period? ANSWER: Yes...QUESTION 9) Will there be someone available to accompany the surveyor after normal business hours. ANSWER: No, however we would be able to issue a grand master key, on a daily basis, to the surveyor, if required. QUESTION 10) Are copies of building functions and a site plan available? ANSWER: Yes, these are available by faxed request to the attention of the Contracting Officer (90C3) at (540) 983-1065. The bid opening date is not extended and remains Wednesday, September 12, 2001 at 2:00 p.m. (local time). A public bid opening will be held in Building 16, Conference Room 3. All other terms and conditions remain the same.
