Description

The VA Salt Lake City Health Care System (VASLCHCS) 500 Foothill Blvd., Salt Lake City, UT 84148 has a requirement for Gelatin Sealed Knitted Vascular Grafts. This notice is a combined synopsis/solicitation for commercial items prepared in accordance with the subparts in Federal Acquisition Regulation 12.6 and FAR part 15 as supplemented with additional information included in this notice. This announcement constitutes the only solicitation; proposals are being requested and a written solicitation will not be issued. Solicitation number Request for Proposal (RFP) 660-080-01 is assigned and shall be referenced on any proposals. All proposals will have name of company, point of contact, telephone number, fax number and may include an e-mail address. This solicitation document and incorporated provisions and clauses are those in effect through Federal Acquisition Circular 97-27. The North American Industry Classification Code is 339113 and the small business size standard is 5 million dollars. STATEMENT OF WORK: The contractor will supply Bifurcated Gelatin Sealed Knitted Vascular Grafts sizes: 12X6, 14X7, 16X8, 18X9, 20X10 22X11 24X12 by 45 cm length, Straight Gelatin Sealed Knitted Vascular Grafts sizes:(6,7,8,10,12,13,16) mm by 15 cm, (18,20,22,24) mm by 12.5 cm, (6,7,8,10,12,14,16) mm by 30 cm, (18,20,22,24) mm by 25 cm, (6,7,8,10,12,14,16) mm by 60cm, (18,20,22,24) mm by 50cm, (6,7,8) mm by 40cm and Cardiovascular Gelatin Knitted Fabric Patches sizes: 6X75 mm, 8X75 mm, 10X50 mm, 10X75mm, 15X50mm, 10X100mm, 15X75mm to the VA Salt Lake City Health Care System. Estimated quantities per year are as follows: Bifurcated Grafts 21 per year, Straight Grafts 13 per year and Cardiovascular patches 4 per year. TERMS OF THE CONTRACT: The proposal shall include a schedule of pricing for each of the grafts listed above in the following format: Description, Unit of Issue, and Unit Price for each of the following time periods: Base Year; October 1, 2001 through September 30, 2002, Option Year One; October 1, 2002 through September 30, 2003, Option Year Two; October 1, 2003 through September 30, 2004, Option Year Three; September 1, 2004 through October 30, 2005 and Option Year Four; October 1, 2005 through September 30, 2006. DELIVERY REQUIREMENTS: Each graft will be ordered by the VA Salt Lake City Health Care System according to commercial practices for ordering and delivering with FOB destination. EVALUATION FACTORS: The following three factors will be used by the VA Salt Lake City Health Care System to determine acceptability; 1) Must be constructed of Knitted Dacron 2) Must be sealed with gelatin, in order to retain topical antibiotics 3) Experimental and clinical data must be available to support the use of this graft when impregnated with antibiotics and placed in an infected or potentially infected bed. Offerors shall provide a complete set of provisions with proposal found at FAR 52.213-3, Offeror Representation and Certifications-Commercial Items. Provisions and clauses found at 52.212-1, 52.212-4, 52.212-5, 52.217-8, and 52-217-9 also apply. Offerors may find the FAR provisions on the Internet at www.arnet.gov/far.
