Description

The Department of Veterans Affairs Domiciliary, 8495 Crater Lake Highway, White City, Oregon is seeking potential contractors to provide Residential/Adult Foster Care services to eligible veterans in the following counties of Oregon: Jackson, Josephine, Klamath and Lake. This notice is intended issuance of Request for Proposal (RFP) 692-15-01 for Residential/Adult Foster Care Services. The contractor shall perform services at the contractors site, which shall include all labor and materials to perform the type of services required within the contract. Offerors must have places of business within the county that they are submitting a proposal on. This requirement is for a base period, plus two (2) one-year options, pending availability of funds. Contract performance will be from dated of award through September 30, 2002. This acquisition is unrestricted for full and open competition. NAICS code 623311/$5 million applies. Multiple award indefinite delivery contracts will be issued according to the requirements within the defined geographical areas. Delivery orders under the awarded contracts can be expected following contract award depending on the number of awards made. Contractors must be those who are licensed providers of Residential/Adult Foster Care. All facilities wishing to participate in this program must accept Medicaid. There will be a meeting for potential contractors wishing to participate in this program, set for Monday, October 1, 2001, at 1PM local time. Meeting will be held at the Dept of Veterans Affairs Domiciliary, 8495 Crater Lake Highway, Building 224 Upper South, Room 251, White City, Oregon 97503.
