Description
This is a combined synopsis/solicitation for commercial items prepared in accordance with the format in Subpart 12.6, as supplemented with additional information included in this notice. This announcement constitutes the only solicitation; proposals are be ing requested and a written solicitation will not be issued. This solicitation is issued as a Request for Proposals (RFP), Solicitation Number DADA13-01-R-0024, using Simplified Acquisition Procedures (SAP) authorized in Subpart 13.5 -- Test Program for Cert ain Commercial Items and is expected to result in the award of one or more firm-fixed-price contract(s). The solicitation document and incorporated provisions and clauses are those in effect through Federal Acquisition Circular 97-27. The solicitation is i ssued as full and open, and all responsible offerors may submit an offer which will be considered. NAICS is 621399. Size Standard is $5,000,000.00. Madigan Army Medical Center (MAMC), Tacoma, Washington, has an immediate need to contract for the personal s ervices of Registered Nurses (RNs) and Licensed Practical Nurses (LPNs) with Medical Surgical experience for the period of 1 Oct 2001 (or date of award if subsequent thereto) through 30 Sep 2002. Shifts will be scheduled by MAMC two (2) weeks in advance (when possible), but are not guaranteed. Estimated total regular time hours for RNs during the stated period is 10,490 HRs (Contract Line Item Number (CLIN 0001); estimated total hours for LPNs required during the stated period is 6960 HRs (CLIN 0002). Es timated overtime hours for RN s is 100 hours (CLIN 0003) and estimated overtime hours for LPN s is 100 hours (CLIN 0004). Guaranteed minimum for each contract awarded under this solicitation is a delivery order for a 13 week period of time. Maximum for eac h contract awarded under this solicitation is delivery orders issued for periods totaling no more than 72 weeks. Delivery orders will be issued by the Western Regional Contracting Office for a not to exceed number of hours for a 13 week period. The Contr acting Officer s Representative will schedule actual shifts with each contractor during that period. Upon completion a modification will be issued definitizing the actual number of hours utilized for that period. The contractor will receive payment only fo r those hours actually worked. Wage Determination No . 1994-2567 Revision No. 20, Date of Last Revision: 07/13/2001 applies to this solicitation as a Technical Exhibit (TE-1) and is hereby incorporated into this solicitation and will be made a part of any resulting contract(s). A copy of the aforementioned w age determination will be provided upon written request. The following Defense FAR Supplement (DFARS) provisions and clauses apply to t his solicitation and are incorporated by reference: DFARS 252.212-7000, Offeror Representations and Certifications-Commercial Items, and DFARS 252.212.7001, Contract Terms and Conditions Required to Implement Statutes or Executive Orders Applicable to Defe nse Acquisitions of Commercial Items. Within DFARS 252.212-7001(a), the following clauses apply: DFARS 252.205-7000, Provision of Information to Cooperative Agreement Holders and DFARS 252.243-7002, Requests for Equitable Adjustment. Potential contractors shall provide a proposal on company letterhead for each item shown above. Pricing should be FOB destination. Also include your firm s prompt payment terms and completed copies of provisions FAR 52.212-3 and DFARS 252.212-7000. Basis for Award: (1) Offers will be evaluated based on submission of Evidence of Availability of Qualified Staff. Offerors shall provide credentials for each person propo sed to perform services at Madigan Army Medical Center (MAMC) for evaluation and approval. (2) Past Performance-Provide references with whom recent past performance can be verified. Include the contract number, dollar value, name, title, phone number o f a person with whom to speak regarding your performance, and a summary of the scope of work. You may submit any other information regarding specific contracts that you deem relevant to the evaluation of your firm's past performance. Information regardin g past performance that is obtained from other sources may be considered by the Government as part of the evaluation process. (3) Price. The Government intends to evaluate offers and award without discussions. However, the Government reserves the right to conduct discussions if determined to be in its best interest. Proposals shall include the RFP number, total number of hours that the offeror can fill, and the hourly rate for RN s and LPN s for both regular time as well as overtime. MULTIPLE AWARDS -The Government reserves the right to make award to multiple contractors on all CLINs if, after considering the additional administrative costs, it is in the Government's best interest to do so. For example, award would be considered to an otherwise acceptable contractor who could provide qualified, acceptable personnel to work less than the maximum hours listed for a particular CLIN, but does not have sufficient personnel available at this time to cover the total hours. ALTERNATE PROPOSALS -- Offeror's are enco uraged to submit alternate proposals detailing their capability to perform in relation to the stated scope of work. The requirement for submission of credential s remains the same.
