Description

This is a combined synopsis/solicitation for commercial items prepared in accordance with the format in FAR Subpart 12.6 as supplemented with additional information included in this notice. This announcement constitutes the only solicitation. Quotations are being requested and a written solicitation will not be issued. The simplified acquisition procedures of FAR Part 13 will be used in conjunction with the commercial item procedures of FAR Part 12. The solicitation is being issued as a Request For Quotation (RFQ). The solicitation number is RFQ 652-012-02. The solicitation document and incorporated provisions and clauses are those in effect through Federal Acquisition Circular 97-27. The Hunter Holmes McGuire VA Medical Center, Richmond, Va. intends to solicit quotations and award a firm fixed price contract on a competitive basis to the offeror whose quotation meets the requirements of this solicitation. Quotations are due by close of business September 14, 2001, local time. Quotations shall be submitted on an "all or none" basis. Partial quotations will be rejected. Please submit quotes on 1)Radiofrequency Lesion Generator with discTrode capability; 2)Sluijter-Mehta kit; 3)SMK disposable cannula 54mm,5mm tip,22 gauge, box of 10; 4)SMK disposable cannula 100mm, 5mm tip, 22 gauge, box of 10; 5)Disposable grounding pad. Quotations must include 1)Unit cost of each item; 2)descriptive literature and product number for each item offered; 3)Delivery time, F.O.B. point, shipping weight and delivery costs. Award will be made to the offeror whose offer conforming to the solicitation will be most advantageous to the Government, price and other factors considered. The government intends to evaluate offers and award without discussions. However, the government reserves the right to conduct discussions later if determined necessary by the Contracting Officer. Quotations shall be submitted to the address listed herein or faxed to 804-675-5588 and directed to the attention of Benjamin Perry. Solicitation Number RFQ 652-012-02 shall be listed on the outside of responses for identification purposes.
