Description
DESCRIPTION: The VA Salt Lake City Health Care System (VASLCHCS), 500 Foothill Blvd, Salt Lake City, UT 84148, has a requirement for cytopathology services. This notice is a combined synopsis/solicitation for commercial items prepared in accordance with the subparts in Federal Acquisition Regulation 12.6, as supplemented with additional information included in this notice. This announcement constitutes the only solicitation; proposals are being requested and a written solicitation will not be issued. Solicitation number Request For Proposal (RFP) 660-056-01 is assigned and shall be referenced on any proposals. This solicitation document and incorporated provisions and clauses are those in effect through Federal Acquisition Circular 97-27. The North American Industry Classification Code is 621111 and the small business size standard is 5 million dollars. STATEMENT OF WORK: The VA Salt Lake City Health Care System (VASLCHCS) will provide the following services in support of this contract: 1. All technical work (cytoprep but no screening) Monday through Friday, 8 AM to 4 PM for all non gynecological VASLCHCS cases. a. VASLCHCS has a 0.5 FTE histotech to prepare non-gyn samples. b. Cytology lab for preparation of samples is established. c. Login of all VASLCHCS cytology samples. The contractor will provide: 1. A cytopathologist (board certified in anatomic pathology) to read all non-gynecological VASLCHCS cases and render a report by FAX or courier within a three-day period. 2. Quality assurance programs which will meet VA regulations, CHAP and JCAHO policies. 3. Technical cytopreparation services as needed for STAT evening and weekend cases such as BAL's. 4. FNA services at the contractor's institution as necessary with scheduling set up through the contractor's offices. .CONTRACT LINE ITEMS: 0001, Base Year, 1300 Cases per year; 0002, Option Year 1, 1300 Cases per year; Option Year 2, 1300 Cases per year; Option Year 3, 1300 Cases per year; Option Year 4, 1300 Cases per year. Submit pricing for all line items. TERM OF THE CONTRACT: This contract will be effective from the date of award for one year with the option to extend for four additional option years. PERSONNEL POLICY: The contractor will be responsible for providing the following: Workers compensation, Professional liability insurance, Health examinations, Income tax withholding and Social Security payments. 3 QUALIFICATIONS: The cytopathologist shall be board certified in anatomic pathology. He/she must have specialty training and be qualified for certification in cytopathology. The qualifications of testing personnel should be included in the technical proposal submitted in response to this solicitation. WORKDAYS: The workhours and holidays are identified as follows: (the services covered by this contract shall be required on national holidays by coverage for emergencies on call) Regular workday and hours: Monday through Friday, 8 AM to 5 PM. After workhours will be: Monday through Friday, 5 PM to 8 AM. National holidays: New Year's Day, Martin Luther King Day, Presidents' Day, Memorial Day, Independence Day, Labor Day, Columbus Day, Veterans Day, Thanksgiving Day, and Christmas, or any other day specifically declared by the President of the United States to be a national holiday. RECORD-KEEPING SYSTEM: The cytopathologist will report directly to the Director of Pathology, VASLCHCS and laboratory medicine service. The cytopathologist must establish a record-keeping system of the number of cases performed. QUALIFICATIONS: Physicians performing work on this contract must be certified in clinical and anatomical pathology. Physicians must be licensed in a state, territory, or commonwealth of the United States or the District of Columbia. The qualifications of such personnel shall also be subject to review by the Director of Pathology and the approval by the VA Facility Director. EVALUATION: The following evaluation factors will be utilized in evaluation of proposals: Technical (documentation required) includes: Capacity; JCAHO or CHAP accreditation; Qualifications of testing personnel; Interaction with Clinicians; and Turn Around Time. Past Performance (documentation required) includes: Previous contracts (provide contract numbers, dates and points of contact), and Price. Technical and past performance, when combined, are more important than price. All potential offerors shall be available to perform the requirements of this contract immediately. Offerors shall provide a complete set of certifications with proposal found at FAR 52.212-3, Offeror Representation and Certifications-Commercial Items. This solicitation incorporates the following clauses: 52.212-1, Instructions to Offerors-Commercial Items; 52-212-2 Evaluation of Commercial Items; 52.212-3, Offeror Representations and Certifications, Commercial Items, WHICH MUST BE COMPLETED AND SUBMITTED ALONG WITH PROPOSAL. In accordance with 52.212-4, Contract Terms and Conditions, Commercial Items, FAR 52.212-5, the following clauses are incorporated by reference in paragraph (b); 52.222-26, Equal Opportunity(EE11246); 52.222-35, Affirmative Action for Special Disabled Veterans and Veterans of the VietNam Era(38USC 4212); 52.222-36, Affirmative Action for Handicapped Workers(29 USC 793); 52.222-37, Employment Reports on Disabled Veterans and Veterans of the VietNam Era; 52.232-33 Payment by Electronic Funds Transfer, Central Contractor Registration(5/99). ALSO INCLUDED IS FAR 52.237-7, Indemnification and Medical Liability Insurance (1/97). Contractor will maintain liability insurance issued by a responsible insurance carrier of not less than the following amount per specialty per occurrence: $1,000,000. ALSO INCORPORATED ARE FAR 52.217-8 (allowing for 30 days notice), 52.217-9 (allowing for 30 days notice) and 852.210-77. Offerors may find the FAR provisions on the Internet at www.arnet.gov/far.
