Description
(i) This is a combined synopsis/solicitation for commercial items prepared in accordance with the format in Subpart 12.6 of the Federal Acquisition Regulation, as supplemented with additional information included in this notice. This announcement constitutes the solicitation; proposals are being requested and a written solicitation is attached. (ii) This solicitation is issued as a Request for Quotation (RFQ) Solicitation Number F64605-01-T-7041. =20 (iii) The solicitation document and incorporated provisions and clauses are those in effect through Federal Acquisition Circular 97-27. (iv) This acquisition is 100% set-aside for small business. The associated NAICS code is 81311 with a small business size standard of $3,500,000. The associated NAICS code is 81311. (v) This synopsis/solicitation contains four contract line item numbers (CLINs) CLIN 0001, 1001, 2001, 3001 and 4001: Contractor to furnish all labor, tools, materials, supplies and transportation necessary to provide Music Coordinator services for all Protestant worship services at Hickam AFB chapels. Price will be submitted as follows: *Services, non-personal to provide music coordination services for Protestant services at the Hickam AFB chapels IAW the attached Statement of Work. CLIN 0001 ? (vi) Facility work is to be accomplished at buildings 1750 (Chapel Center) and 500 (Nelles Chapel) on Hickam AFB, Hawaii. Performance Period: 1 October 2001 to 30 September 2002.=20 Purpose: To provide music coordination services for the Hickam AFB chapels.=20 Scope: STATEMENT OF WORK FOR PROTESTANT MUSIC COORDINATOR Music Coordinator (hereafter referred to as ?Contractor?) shall provide all coordination of all Protestant Worship Services as the Hickam AFB, HI Chapels (Chapel Center, Bldg 1750 and Nelles Chapel, Bldg 500). DESCRIPTION OF SERVICES REQUIRED 1. SCOPE OF WORK The Contractor shall provide all Protestant Music Coordination through the Senior Protestant Chaplain and with all assigned Protestant Chaplains of the following Sunday Worship Services: 0845 (Contemporary Service), 0845 (Traditional Service), and 1200 (Gospel Service). The Contractor will ensure that the highest possible quality of music is compatible with the worship experience of each Protestant Worship Service. Subcontracting positions of Music Director and Musician will be provided and coordinated by the contractor for each of the worship services. The Contractor will provide a statement of work to the subcontractors. A total of 6 subcontractors positions shall be coordinated for each Sunday and special occasions. Performance shall be in accordance with the established procedures and standards of the 15th Air Base Wing/HC and the scope of this Performance Work Statement. 2. PERSONAL The Contractor shall ensure that services are performed in a professional manner. Contractor shall have at least five years of experience working with the military or other federal agencies, either as a paid or volunteer status. Contractor shall have at least five years of music experience and music leadership experience in a church worship setting. 3. TIME AND PERFORMANCE Coordination for worship services may be conducted by way of telephone. Actual hours of work may be amended as required to accomplish the mission. The Contractor or his/her representative (a volunteer position) will attend the monthly Protestant Parish Council meeting to inform the status of the Protestant Music Program. 4. FACILITIES AND EQUIPMENT The Contractor shall have access to the Chapel administration equipment (i.e. Copier, computers, fax machines, etc.) to conduct his/her coordination of this contract. The Contractor may use musical instruments in the chapel facilities or may use personal property as required. 5. PERIOD OF CONTRACT This contract begins 1 October 2001 and ends on 30 September 2002. Either party may terminate it upon 30 days notice in writing. Either party for default by the other party in any of the obligations stated herein may also terminate it. 6. METHOD OF PAYMENT The Contractor will request each Protestant Chaplain of the respective worship service to verify work accomplished by the subcontractors. Verification will then be completed by the Senior Protestant Chaplain, Wing Chaplain and the Cost Center Manager. Monthly invoices will then be submitted. Partial payment is authorized. Upon payment, contractor will pay the subcontractors, and substitutes for subcontractors. Documentation for income tax purposes will be provided by the contractor for subcontractors and their substitutes as needed.=20 7. UNACCEPTABLE PERFORMANCES If the contractor continually provides sub-standard performances, the chaplain in charge of the worship service will review that unacceptable performance with the contractor and the Senior Protestant Chaplain before any action is taken. 8. ACCEPTANCE OF SERVICES Acceptance of services shall be performed by the Wing Chaplain or designated representative by certifying an AF Form 1449 or DD Form 250 that subject services have been rendered and are acceptable during the month for which payment is requested. 9. CHARGES Weeks: 52 Hours per week: As required to accomplish the contract SPECIFIC REQUIREMENTS The Contractor will: 1. Provide all music direction and pianist accompaniment for each Protestant Worship Service. 2. Attend or have a volunteer representative at each Protestant Parish Council monthly meeting (usually the third Friday of the month, from 1130-1300). 3. Submit the monthly music director and accompanist service and rehearsal completions in a timely manner. 4. Coordinate a Christmas and Easter Cantata each year. 5. Provide coordination for all substitutes for subcontractors and pay their honorariums as agreed upon by the contractor and subcontractor. 6. Coordinate with the Senior Protestant Chaplain any needs of the contractor and the needs of the congregations of the Protestant program. 7. Be supportive of the coordination of special music programs and services, if possible. The Contractor will possess the following qualifications: 1. Experience with volunteer recruitment, training, and management. 2. The ability to coordinate with the Base Chaplains, Staff, Protestant Community volunteers and subcontractors serving in this contract. 3. General knowledge of the workings of the Air Force at Hickam AFB, HI and the military way of life. 4. Experience in producing and maintaining documentation of training, publicity, and financial expenditures. 5. Experience and ability to coordinate schedules with subcontractors and chapel volunteers. 6. Proficiency in writing, public speaking, and public relations. (vii) All work to be performed will be delivered and accepted at buildings 1750 and 500 on Hickam AFB, Hawaii during the hours stated above. =20 (viii) The provision at FAR 52.212-1, Instructions to Offerors ? Commercial, applies to this acquisition without addenda. (ix) A firm fixed price purchase order will be issued based upon the proposal determined to provide the best value based upon price and past performance. (x) Offerors shall include a completed copy of the provision at FAR 52.212-3, Offeror Representations and Certifications ? Commercial Items, with offer. (xi) The clause at FAR 52.212-4, Contract Terms and Conditions ? Commercial Items, applies to this acquisition without addenda. (xii) The clause at FAR 52.212-5, Contract Terms and Conditions Required To Implement Statutes Or Executive Orders ? Commercial Items, applies to this acquisition.
