- This is a combined synopsis/solicitation for commercial items prepared in accordance with the format in FAR subpart 12.6 as supplemented with additional information included in this notice. This announcement constitutes the only solicitation; an offer is being requested and a written solicitation will not be issued. The solicitation number is F22600-01-R-A019 and is being issued as a request for quotation. The solicitation document and incorporated provisions and clauses are those in effect though Federal Acquisition Circular 97-27and Defense Federal Acquisition Regulation Supplement Change Notice 20001213. This acquisition is 100% set-aside for small business concerns. The NAICS code is 621991 and the small business standard is $7.5 million. All responsible small business sources may submit a proposal or quotation which shall be considered. Keesler AFB intends to issue a firm-fixed price contract to purchase the following BLOOD PRODUCTS: leukoreduced red blood cells ? 50 units; leukoreduced apheresis platelets ? 60 units; random donor platelets ? 100 units; and fresh frozen plasma ? 95 units. Authorized staff members of the Keesler Medical Center Blood Bank will place calls under the terms of this contract. The Contractor shall deliver products within 24 hours of notification by authorized Center personnel. Delivery date: 30 Sep 01. Deliver to: 81 MDG/SGQC. FOB ? Destination. Inspection & acceptance: Keesler AFB, MS 39534. Clauses and Provisions reference may be accessed electronically at this address: http://farsite.hill.af.mil/ The following Federal Acquisition Regulations (FAR) provisions and clauses apply: 52.211-15 , Defense Priority and Allocation Requirements. This is a DO rated order with a priority identification symbol of C9; 52.212-1, Instructions to Offerors-Commercial Items is amended to read: submit signed and dated offer or most current price list on Company Letterhead to 81 CONS/LGCB , 310 M Street, Room 102, Keesler AFB MS 39534-2147 at or before 1500 (3:30pm) CST 7 Sep 01. Quotation may be submitted via FAX (228) 377-3298 or e-mail to tom.kocurek@keesler.af.mil . Addenum to 52.212-1: The following paragraphs of this clause are tailored as follows:(c) Period of Acceptance of Offers. The offeror agrees to hold the prices in its offer firm for 60 calendar days from the date specified for receipt of offers, unless another time period is specified in an addendum to the solicitation.(f) Late Offers. Offers or modifications of offers received at the address specified for the receipt of offers after the exact time specified for receipt of offers may not be considered. In accordance with 52.212-2, Evaluation-Commercial Items, the Government will award a contract resulting from this solicitation to the responsible offeror whose offer is determined to be the most advantageous to the Government ? price, past performance and ability to deliver required quantities in required timeframes considered. All factors are of equal weight in the determination. Each offeror must submit a completed copy of the provision at FAR 52.212-3 -- Offeror Representations and Certifications?Commercial Items with their quote. FAR 52.212-4 Contract Terms and Conditions?Commercial Items is incorporated with the following paragraphs tailored as follows: (a) Inspection/Acceptance. Inspection and acceptance of the services will be performed at Keesler Air Force Base, MS, by 81 MDSS/SGSLC. (c) Changes. Changes in the terms and conditions of this contract may be made only by written agreement of the parties with the exception of administrative changes such as changes in the appropriations data, paying office, etc., which may be changed unilaterally by the Government. FAR 52.212-5 Contract Terms and Conditions Required to Implement Statues or Executive Orders-Commercial Items (Apr 2001) (Deviation) however, for paragraph (b) only, the following clauses apply: FAR 52.222-26, Equal Opportunity; FAR 52.222-35, Affirmative Action for Disabled Veterans and Veterans of the Vietnam Era; and, FAR 52.222-36, Affirmative Action for Handicapped Workers with Disabilities; and DFARS 252.212-7001(Deviation). The following additional clauses are incorporated into the solicitation: 52.216-18, Ordering; 52.216-19, Order Limitation; 52.216-21, Requirements; 52.219-6, Notice of Total Small Business Set-Aside; 52.222-3, Convict Labor; 52.225-13, Restrictions on Certain Purchases; 52.232-33, Payment by Electronic Funds Transfer-Central Contractor Registration, DFARS 252.204-7004, Required Central Contractor Registration, and DFARS 252.225-7001, Buy American Act-and Balance of Payments Program. Include with quote at least 2 references of past performance to contact. Total Quantity is an estimate since exact price has not been established at the issue of this solicitation.
