Description

This is a combined synopsis/solicitation for commercial item prepared in accordance with the format in Subpart 12.6, as supplemented with additional information included in this notice. This announcement constitutes the only solicitation. Quotes are bein g requested and a written solicitation will not be issued. This requirement is being issued under the Simplified Acquisition Procedures (SAP) as a Brand Name or Equal in accordance with the Federal Acquisition Regulation (FAR) 52.211-6. The solicitation document and incorporated provisions and clauses are those in effect through Federal Acquisition Circular (FAC) 97-27 (25 June 01) and Defense Federal Acquisition Requlation Supplement (DFARs), 1998 edition, and current to DCN 20001213. It is anticipated that payment will be made by Government Visa Credit Card. The North American Industry Classification System (NAICS) is 334517 with size standard of 500 employees. The manufacture is Lorad, 36 Apple Ridge Rd, Danbury, CT 06810. The quote shall consist of Contract Line Item (CLIN) 0001,Model LPX-160 (digital) air-cooled x-ray unit, 160kv 5ma. Continuos duty cycle, 40 degree cone of radiation, 1.5mm effective focal spot size, .030 inches beryllium window tube, 110/220 volt 50/60 HZ input. Unit consists of the following: Control Unit PN#3-000A-0738, Tube Head PN#3-000A-0768, Control Cable PN#1-040A-0342, Power Cable PN#1-040A-0341, Power Cable (Fan) PN#1-040A-0355, Operator/Maintenance Manual. Options: Tubehead Tripod PN#3-000-0754, Laser Pointer PN#3-000- 0792, Adapter for Laserpointer PN#9-200-0327. All quotes shall be clearly marked with this Solicitation number DATM07W81R8T12400001 and e-mailed to jackie.wengstrom@yuma.army.mil or Fax 928-328-6849 no later than Close of Business, Mountain Standard Time (MST) 5:00 P.M., 7 September 01.Offeror that fail to furnish required representations or information as required by 52.212-1, 52.212-3, and 252.212-7001 cited below, or reject the terms and conditions of this solicitation may be excluded from consideration .
