Description

Request for Quotation, Sol. 544-091-01 due date of 9/13/01, 2:00 p.m. local time, is hereby issued for commercial items to be used by the VA and is prepared in accordance with the format in Federal Acquisition Regulation (FAR) Subpart 12, Acquisition of Commercial Items. NAISC is 621111. The WJBD Veterans Affairs Medical Center has a need for Radiation Therapy for beneficiaries of the Department of Veterans Affairs Greenville Outpatient Clinic, Greenville, SC. Radiation Therapy to be furnished by a contractor with office (s) within a 20-mile radius of the VA Greenville Outpatient Clinic, Greenville, SC. The services must be performed by physicians who are Board Certified in Radiation Oncology Medicine and Hematology/Oncology. The contract will be for one-year effective October 1, 2001 through September 30, 2002. The contractor will be required to provide the following services to include but not limited to: Radiation Therapy, initial consultations, clinic treatment planning, medical radiation, physics, dosimetry, treatment devices, special services and clinical treatment management. Attachments B and C to the solicitation must be obtained from the contracting officer by a written request faxed to (803) 695-6857.
