- Description
- Provide preventive maintenance, calibration, and all intervening service calls on lea ATL model Ultramark 9 and lea ATL HD13000CV Ultrasound Systems. This is a combined synopsis solicitation for commercial items prepared in accordance with the format in Subpart 12.6, as supplemented with additional information included in this notice. THIS ANNOUNCEMENT CONSTITUTES THE ONLY SOLICITATION; PROPOSALS ARE BEING REQUESTED AND A WRITTEN SOLICITATION WILL NOT BE ISSUED. Solicitation is being issued as a request for proposal (RFP). The solicitation and incorporated provisions and clauses are those in effect through Federal Acquisition Circular (FAC) 97-27. The proposed procurement is unrestricted. The North American Industry Classification System (NAICS) code is 811219. Small business, women-owned and small disadvantaged business concerns are encouraged to submit offers. A copy of the bid schedule, statement of work, addendum to clauses, and SF 1449 (complete blocks 12, 17, 30 and submit with proposal) are located in the electronic posting system. Place of delivery and acceptance is Maxwell AFB AL 36112. FOB is destination. Period of performance is 01 Oct 01 through 30 Sep 02 with four (4) one year option periods. Funds are not presently available for this solicitation. The Government?s obligation under this solicitation is contingent upon the availability of appropriated funds from which payment for contract purposes can be made. No legal liability on the part of the Government for any payment may arise until funds are made available to the contracting officer for this contract and until the contractor receives notice of such availability, to be confirmed in writing by the contracting officer. Pricing that is reflected in the proposal shall remain valid form the date proposed to contract expiration. Only written requests for a copy will be accepted. The following Federal Acquisition Regulation (FAR) clauses apply to this solicitation and can be viewed through Internet access at the Air Force FAR site http://farsite.hill.af.mil or http://www.arnet.gov. The provision at FAR 52.212-1, Instructions to Offerors?Commercial, applies to this acquisition and addendum; Paragraph (b), Submission of offers, add: subparagraph (12), Provide a copy of factory training certificate(s) or documentation for all employees to provide maintenance and complete FAR clause 52.222?48 in addenda to 52.212-4; Paragraph (c), Period of acceptance is changed from 30 days to 90 days from the date proposed to 30 Sep 02; Paragraph (e), Multiple offers and (h), Multiple awards are deleted, Add paragraph (k); The following provision is added: FAR 52.217-5, Evaluation of Options (Jul 1990). Price will be the basis for proposal evaluation. Offerors shall include a completed copy of he provision at FAR 52.212-3, Offeror Representation and Certifications-Commercial Items (offerors must include a completed copy of this provision with their proposal). The clause at FAR 52.212-4, Causes Incorporated by Reference (web addresses to access clauses and provisions are: http://farsite.hill.af.mil or http://www.arnet.gov/far/). The clause at FAR 52.212-5, Contract Terms and Condition Required to Implement Statutes or Executive Orders?Commercial Items applies to this acquisition. Under FAR clause 52.212-5, the following are applicable (b) 11, 12, 13, 14, 15, 16, 21, 24, and (c) 1, 2, 3, DFARS 252.212-7001, Contractor Terms and Conditions Required to Implement Statutes of Commercial Items, the following is applicable 252.225-7001 Buy American Act Balance of Payments Program; addendum DFARS 252.204, Required Central Contractor Registration. All offerors must be registered in the Central Contractor Registration (CCR) system database prior to receiving an award.
