- This is a combined synopsis/solicitation for commercial items prepared in accordance with the format in FAR, Subpart 12.12 as supplemented with additional information included in this notice. This notice constitutes the only solicitation; bids are being requested and a written solicitation will not be issued. The solicitation document and incorporated provisions and clauses are those in effect through Federal Acquisition Circular number 97-27. The NAIC code for this solicitation is 811310. The small business size standard is 5.0 million. The contractor shall provide labor, material, equipment, and supplies necessary for a full service preventive maintenance and service contract on the following Government-owned equipment: Electron Microscope, Model: MACM10-SN# D838, Gatan TV System-Model MA5201 S/N# D838.1. The SCHEDULE OF PRICES: Base Year (10/1/01-9/30/02) $________per month x 12 months @ $________per year. Option Year 1 (10/1/02-9/30/03) $_________per month x 12 months @ $________per year, Option Year 2 (10/1/03-9/30/04) $__________per month x 12 months @ $_________per year for a grand total of $______________________. The Vendor will provide all labor, tools, travel, equipment, and parts required to perform repair and one (1) preventive maintenance (PM) on equipment covered by the contract. All equipment shall be maintained in proper operating condition per the model specific manufacturer's service manual. Only original equipment manufacturers (OEM) parts, manuals, and schematics will be utilized to perform the service. These must be available on site at time of service. All defective parts replaced become the property of the vendor.All work will be performed during normal working Monday through Friday hours 8:00 a.m. to 4:30 p.m. (excluding government holidays). Contractor shall provide a phone call back by a qualified technician within two (2) hours of a service request and on-site response by a qualified technician within four (4) hours of original request for service. Contractor will report to Biomedical Engineering upon arrival during normal business hours. Vendor repair personnel must be manufacturer trained to work on the specific equipment listed in the contract and must submit a certificate of training prior to commencing work on this contract. Vendor will provide all software and hardware updates upon release at no additional cost to the Government throughout the life of this contract. Software and hardware updates are limited to those items which are commercially available from the OEM. Field service reports shall be provided at the time of the service call and must be signed by the Contracting Officer or designee and filed with the Biomedical Engineering Department. Field service reports shall be detailed as repair, performance, procedures, calibrations, parts and labor provided.Preventative maintenance shall follow the manufacturers specification and include electrical safety testing, lubrication, adjustment, calibration, testing and replacement of faulty parts. LOCATION OF AUTHORIZED BRANCH OR SERVICE DEPARTMENT: Submit name, address and phone number to be used for emergent repair and scheduled repairs.Payments will be made monthly in arrears. Add Option:The vendor will provide two week training costs and expenses to include a round trip air fare from Washington, DC to New York, NY, hotel in Stanford, meals, training cost, and round trip ground transport from New York to Stanford. The two week training course consists of (2) five day sessions ( 12 actual days). Please include in bid package a completed Standard 1449 which can be found at www.gsa.gov/forms/farnum, required provisions, and a schedule of prices for the base and (2) two option years. Acceptance time will be 60 days from bid opening date. The following provisions apply to this solicitation: 52.212-1 Instructions to Offerors-Commercial, 52.212-3 Representations and Certifications- Commercial Items, 52.214-4 Contract Terms and Conditions-Commercial Items, 52.219-2 Equal Low Bids, and 52.212-5 Contract Terms and Conditions including Clauses incorporated by reference under Section (b) (6) 52.222-26 Equal Opportunity (EO 11246), 52.222-3 Affirmative Action for Special Disabled and Vietnam Era Veterans (38 USC 4212), 52.222-36 Affirmative Action for Handicapped Workers (29 USC 793), and 52.225-3 Buy American Act-Supplies (41 USC 10). Bidders responding to this announcement shall submit a bid along with the information required in 52.212-1 and a completed copy of provision 52.212-3 and VAAR 852.219-70 Veteran Owned Small Business (Dec 1990) to VA Medical Center (688/90C), Room 1B-112, 50 Irving Street, NW, Washington, DC 20422 by 9/12/01 at 4:30pm Local Time. A written notice of award will be mailed or otherwise furnished to the successful bidder within the time for acceptance specified in the bid shall result in a binding contract without further actions by either party.
