Description

This is a combined synopsis/solicitation for commercial items prepared in accordance with the format in FAR subpart 12.6, as supplemented with additional information included in this notice. This announcement constitutes the only solicitation. A quote is being requested and a written solicitation will not be issued. Solicitation number F7COM12110200 is hereby issued as a Request for Quote. The incorporated provisions and clauses are those in effect through Federal Acquisition Circular 97-27. This acquisition is restricted to total small business and is under North American Industry Classification System code 334220 with a small business size standard of 750 employees. This requirement is for PAGER SERVICE, 380 total. All 380 pagers need to be in one zone that covers all of South Western Oklahoma to include Oklahoma City, Tulsa, Stillwater, El Dorado, Clinton, and Wichita Falls, Texas. The access number for the pager system should be a local number. The services rendered should start on 1 Oct 01 and end on 31 Sept 02. Currently, Altus AFB has the 380 pagers split into five group calls. Group one and two are for the 97th Med Group with 28 pagers. The third group is for the 97th Civil Engineering Squadron with 10 pagers. The fourth goes to CES/CEF with 40 pagers. The fifth group goes to the DCG with 19 pagers. The following clauses and provisions are incorporated and are to remain in full force in any resultant purchase order: FAR Clause 52.212-1, Instructions to Offerors -- Commercial Items (Oct 2000). It is incorporated by reference and applies to this acquisition. It is amended to read: Submit signed and dated offer to 97 CONS/LGCA, 205 S. 6th St. Bldg. 318, Altus AFB OK 73523-5147 at or before 4:30 PM CST, 7 Sep 01. Submit signed and dated quotations on company letterhead or quotation form. Quotations may be faxed to (580) 481-7472. Offers on this acquisition shall be evaluated on the basis of price factors only. The government reserves the right to make award on the initial quote without discussions. Each offeror shall include a completed copy of the provisions at FAR clause 52.212-3, Offeror Representations and Certifications-Commercial Items (May 2001) with the quotation. If a copy of the provisions are needed, please request one from Amp Phommachanh and a copy will be faxed. FAR clause 52.212-4 (May 2001), Contract Terms and Conditions-Commercial Items, is incorporated by reference and applies to this acquisition with the following addendum: DFARS 252.204-7004 Required Central Contractor Registration (Mar 2000). The clause 52.212-5, Contract Terms and Conditions Required to Implement Statutes or Executive Orders-Commercial Items (May 2001) and its deviations (May 2001) applies to this acquisition and is incorporated by reference. A Firm-Fixed Price purchase order will be issued in writing. No telephonic responses will be processed. Department of Labor Wage Determination 94-2525 Rev. 20 applies and is incorporated into this Solicitation and any resultant Purchase Order.
