Description

This solicitation for commercial items is prepared in accordance with FAR Part 13 and the format in FAR Subpart 12.303, as supplemented with additional information included in this notice. This announcement constitutes the official notification for which quotes are being requested and a written solicitation will be issued. The solicitation document Request for Quote (RFQ) 187-014-1 and incorporated provisions and clauses are those in effect through Federal Acquisition Regulation Circular 97-27. This procurement is being advertised as a fully competed procurement, under North American Industry Classification System (NAICS) code 621111 with a size standard of $5.0 million. The Federal Bureau of Prisons, Federal Correctional Institution (FCI), Phoenix, AZ intends to award a contract for Obstetrics and Gynecology services to the contractor who submits the best value offer. The services required include but are not limited to examination, evaluation, and recommendation of treatment of inmates incarcerated at the Federal Prison Camp as well as pre-natal, delivery, and post-natal services provided at the contract hospital. Services will be required from date of award through 30 September 2002 with four twelve-month option periods, for an estimated twelve (12) four-hour sessions and six (6) to eight (8) deliveries required during a twelve-month period. Anticipated due date for offers is 20 September 2001. Offers in original shall be received at the Federal Correctional Institution, 37900 N. 45th Avenue, Attention: Stefanie Kongable, Phoenix, Arizona, 85086, by the date established in the solicitation, 3:00 pm local time.
