- Description
- This is a combined synopsis/solicitation for commercial items prepared in accordance with the format in Subpart 12.6, as supplemented with additional information included in this notice. This announcement constitutes the only solicitation; proposals are being requested and a written solicitation will not be issued. The solicitation number is RFQ 541-114-01 and is issued as a request for quotation (RFQ). The solicitation document and incorporated provisions and clauses are those in effect through Federal Acquisition Circular 27. This is an unrestricted procurement. The North American Industry Classification System (NAICS) code is 421450, with a size standard of 100 employees. Item: 0001, (WAG2VC17ASA) Sterilization Container 24"x10 3/4"x 6 3/4" quantity 12 each; 0002 (WAG2V15ASA) Sterilization Container w/ drain 24"x 10 3/4"x 6" quantity 27 each; 0003, (WAG3V15ASA) Sterilization Container 18"x 11"x 6" quantity 26 each; 0004, (WAG1V15ASA) Sterilization Container 12"x10 3/4"x 6" quantity 45 each; 0005, (WGN63P100A) Aluminum Basket, Perforated Aluminum w/ fold down handles 20"L x 9 1/2" W x 4"H quantity 39 each; 0006, (WGN43P100A) Aluminum Basket, Perforated Aluminum w/ fold down handles 15 3/4"L x 9 1/2" W x 4"H quantity 26 each; and, 0007, (WGN33P100A) Aluminum Basket, Perforated Aluminum w/ fold down handles 10"L x 9 1/2" W x 4"H quantity 45 each. These items are manufactured by Medline, substitutions will be accepted. Sterilization Containers shall serve as the sterile storage of instruments and surgical apparatuses. Sterilization Containers shall be filter-less and have a lid and a base and together shall be self sealing and the contents sterilized once the autoclaving process is completed. The base shall have two handles on the opposite ends of the longitudinal dimensions; the lid shall be able to be secured to the base by way of a latch; and, they must be stackable for storage. Sterilization Containers shall a window for placing placards to identify contents and date autoclaved. Aluminum Basket shall be able to fit into Sterilization Containers. The Sterilization Containers must be able to fit into the standard Autoclave Sterilization Containers shall be able to support weights of up to 25 lbs. The Sterilization Containers in 0001, 0002, 0003, and 0004 are filter-less and become storage container for the sterilized surgical instruments with a sterilized shelf life of one year. This container system eliminates the requirement for wrapping major instrument trays. Aluminum Basket are perforated aluminum on the on the base and the sides of the tray. Each Aluminum Basket is inserted in the respective Sterilization Container. Aluminum Basket handles shall be able to fold down for maximum space utilization. Warranty of said product shall be in accordance with the standard industry practice in effect at time of award. Contractor shall submit invoices to Fiscal (04), Louis Stokes Cleveland VAMC, 10000 Brecksville Road, Brecksville, OH 44141. Each item shall be priced separately, by Line Item Number. All items shall be delivered within thirty (30) calendar days after receipt of a purchase order. Delivery shall be F.O.B. Destination. These items shall be delivered to the Louis Stokes Cleveland VA Medical Center, 10701 East Boulevard, Cleveland, Ohio 44106 attention: warehouse. The following provisions apply to this acquisition: 852.219-70 Veteran-Owned Small Business; 852-270-4 Commercial Advertising; 52.212.-1, Instructions to Offerors-Commercial Items applies to this acquisition with the following paragraphs (e) and (h) deleted; 52.212-3, Offeror Representations & Certifications Commercial Items; FAR 52-212-4, Contract Terms & Conditions -- Commercial Items, FAR 52-212-5, Contract Terms & Conditions Required To Implement Statues or Executive Orders-Commercial Items, including clauses incorporated by reference under Section (b)7 through 10.; 52-215-1 Instruction to Offerors-Competitive Acquisition; and, 52.215-5 Facsimile Proposals.
