The projected quantities of requirements are undetermined at this time since this requirement will be funded with FY 02 monies. Part 1 -- Business Information- Company Na me/Address/Point of Contact/CAGE Code/Size of business pursuant to the North American Industry Classification Sys tem (NAICS) code 336413 (1000 Employees). Also indicate whether your company is small, small disadvantaged, wom an-owned small, HUB-Zone, 8(a) certified small business under the above NAICS code. Also indicate whether or not your firm is domestically or foreign owned and whether or not you are currently registered in Central Contractor Registration (CCR). Part II -- Capability -- 1) Describe your facility's capabilities and the nature of the goods and services you provide. Do you have an existing product line that meets or exceeds the Government's needs? 2) Past experience on previous projects similar in complexity to this requirement to include contract numbers and a n agency point of contact. 3) Experience with manufacturing and fabrication of conformal seat cushions. 4) Qualit y assurance processes and test qualification practices your company employs. 5) Established catalog or market pri ces. 6) Any differences between the products offered to the Government and general public. 7) Type of foams used in the construction of conformal seat cushions. 8) Fabric types and colors offered. 9) Compliance with FAA fl otation and flammability standards. 10) Crash testing performed on products. 11) Ergonomic effectiveness of con formal seat cushions. 12) What are your standard procedure warranty policies offered?
