Description

Furnish all labor, materials and equipment to provide Testing Lab Services for construction Project 622-127, Psychiatric Patient Privacy, at the Tennessee Valley Healthcare System, Alvin C. York VA Medical Center, Murfreesboro, TN. Principles of the Testing Laboratory must be Registered Professional Engineers (PE's) with a minimum of five years experience in the types of testing required under this contract. Personnel performing laboratory testing, field testing, and inspection must have not less than one year of full time experience in the duties to be performed under this contract. The contract period will be for the duration of the construction contract performance period of 760 calendar days with an option to extend the contract period due to construction delays or time extensions. Selection will be based on the following criteria: (1) Length of time firm has been in business; (2) Number of previous projects of similar size and nature; (3) Years experience of Principle P.E. for firm; (4) Years of directly related full time experience for technicians that will be used for each procedure; (5) Training courses completed by technicians that will be used on this project; (6) Firm is accredited by a national testing lab organization; (7) Number of positive references available from local clients; (8) Ability to respond to requests for inspection services. This is not a request for proposal. Interested firms should submit up-to-date SF 254 and SF 255 by 4:30 p.m. CST on September 21, 2001.
