Description

This is a combined synopsis/Solicitation for commercial items prepared in accordance with the format in FAR Subpart 12.6 as supplemented with additional information included in this notice. This announcement constitutes the only Solicitation; quotes are being requested and a written Solicitation will not be issued. Solicitation FS-WOC-01-474 is being issued by the USDA Forest Service as a Request for Quotations. This document and incorporated provisions and clauses are those in effect through Federal Acquisition Circular 97-27. This Solicitation will be awarded under a Firm Fixed Price contract based on responses to the Solicitation. The subsequent contract will be awarded for one (1) Base Year and three (3) 1-year options. This Solicitation will not be set aside for Small Business. The North American Industry Classification (NAICS) Code is 235950, Building Equipment and Other Machinery Installation Contractors. This project is to provide maintenance for two (2) side-by-side Otis Electric Passenger Elevators No. 342541-2, 3800-pound capacity each at approximately 288 FPM serving two (2) openings each in line located at Blanchard Springs Caverns, 3-1/2 miles east of Fifty-Six, AR, off State Highway 14 in Stone County. The elevators referenced in this solicitation to comply with all requirements of current American National Standard Safety Code for Elevators, Dumbwaiters, Escalators, and Moving Sidewalks (hereinafter referred to as A17.1 Code) ASE Inspectors Manual and all other applicable laws, regulations, ordinances, codes, etc. The A17.2 Code shall be used as a guide to establish that equipment is operating safely. Item 1: Maintenance of two (2) elevators located at Blanchard Springs Caverns from October 1, 2001, through September 30, 2002 (Base Year). Item 2: Maintenance of two (2) elevators located at Blanchard Springs Caverns from October 1, 2002, through September 30, 2003 (Option Year One). Item 3: Maintenance of two (2) elevators located at Blanchard Springs Caverns from October 1, 2003, through September 30, 2004 (Option Year Two). Item 4: Maintenance of two (2) elevators located at Blanchard Springs Caverns from October 1, 2004, through September 30, 2005 (Option Year Three). Bi-weekly (twice monthly) servicing and inspections shall be performed during regular working hours of regular working days of the trade. Dates and hours shall be scheduled in advance with the Contracting Officers Representative. Should major repairs or adjustments be required constituting down time in excess of 4 (four) hours, the Contracting Officers Representative may require the Contractor to perform the work before or after operating, as to avoid conflicts with scheduled cavern tours. The provisions of FAR 52.212-1, Instructions to Offeror -- Commercial Items; FAR 52.212-4, Contract Terms and Conditions -- Commercial Items; FAR 52.212-5, Contract Terms and Conditions Required to Implement Statutes for Executive Orders -- Commercial Items; and FAR 52.217-9 Option to Extend the Term of the Contract apply to this acquisition. The provision of FAR 52.212-2, Evaluation -- Commercial Items applies and the evaluation factors are past performance and price. A completed copy of the provision in FAR 52.212-3, Offeror Representation and Certifications -- Commercial Items shall be included with the quote.
