Description
This is a combined synopsis/solicitation for commercial items prepared in accordance with the format in Subparts 12.6 of the Federal Acquisition Regulation, as supplemented with additional information included in this notice. This announcement constitutes the only solicitation, and a written solicitation will not be issued. Solicitation 528-12-01 is issued as a Request for Quote (RFQ) and incorporates provisions and clauses in affect through Federal Acquisition Circular 97-27. This solicitation is a small business set-aside. The North American Industrial Classification System code is 811310. The Government intends to award a Firm Fixed Price purchase order under simplified Acquisition Procedures (SAP), and will consider all quotes that are submitted on, or before, July 31, 2001. The contractor shall provide all labor, equipment, and parts to provide PM inspections and full unscheduled maintenance for specified automatic sliding doors at the VAWNY Healthcare Facility in Buffalo, at 3495 Bailey Avenue, Buffalo, New York, 14215, and in Batavia, at 222 Richmond Avenue, Batavia, New York, 14020 for the period August 1, 2001 through July 31, 2002 with two (2) option years. All work performed on this contract shall be done during the hours of 8:00 A.M. -- 4:30 P.M., Monday through Friday, excluding Federal holidays. Contractor shall notify the Contracting Officer's Technical Representative (COTR), Engineering Service, at least seven (7) days prior to each scheduled service visit. Response time for unscheduled work shall be on-site within 24 hours following telephone notification from the VA. After each scheduled service call (within 2 weeks), the contractor shall provide a complete and legible service report to the Maintenance Scheduler's Office, Engineering Service. This report shall identify the location of each door having received a PM inspection, the date of inspection and signature certifying the completion of the inspection. The equipment to be serviced are the automatic door operators, controls, mats, sensors, electric eyes and all other switching devices. The VA will maintain the doors, glass, crash bars, rails, fingerguards, door frames, locks, bolts and other door hardware. Worn or defective parts are to be replaced as part of this service agreement and shall be identified in a field service report (for an unscheduled visit) and in the PM service report for PM work. Parts furnished under this agreement are to be genuine replacement parts specifically designed for use on this equipment. All automatic door operator equipment for the listed door locations shall receive two (2) PM inspections during the contract period. (See inspection report requirements, above.) PMs will consist of total system examination, cleaning, tightening and tune in of all mechanical devices, system testing, door speed and performance testing and adjustment, electrical function analysis, hardware and accessory inspection and lubrication, with emphasis on breakaway features.
