Description

1. This is a combined synopsis/solicitation for commercial items prepared in accordance with subparts in the Federal Acquisition regulation 12.6, as supplemented with additional information included in this notice. This announcement constitutes the only solicitation; quotations are being requested and a written solicitation will not be issued. This solicitation document and incorporated provisions and clauses are those in effect through Federal Acquisition Circular 97-27. 2. Specifications. Based upon this Solicitation No. RFQ 600-137-01, the Government intends to procure a full service maintenance contract for base year plus four (4) option years for the following laboratory equipment at VA Southern Nevada Healthcare System, 1700 Vegas Drive, Las Vegas, NV: (a) Bio-Rad Variant Analyzer, Model # 270-0102, one (1); and (b) Bio-Rad Autoprep Sample Preparation System, Model # 210-0570, one (1). Full service for both equipment items shall consist of (a) unlimited on-site corrective maintenance, with response time within 24 hours of service request, Monday through Friday, 8:00 a.m. to 5:00 p.m., regardless of problem; (b) one (1) on-site corrective PM per year; (c) all parts, labor, and travel included in price; (d) produce hardware enhancements at no additional charge; (e) annual in-service training; and (f) loaner instrument provided within 24 hours at no additional cost if unit must be removed from premises for repair. Quotation shall reference solicitation number and reflect a single, full-service maintenance price for each piece of equipment listed above for each contract year (i.e., Base, OY1, OY2, OY3, and OY4. 3. Clauses. Incorporated herein by reference are the following Federal Acquisition Regulation (FAR) Clauses: 52.212-1: Instructions to Offerors -- Commercial Items; 52.212-3: Offeror Representations and Certifications -- Commercial Items; 52.212-4: Contract Terms and Conditions -- Commercial Items; 52.212-5: Contract Terms and Conditions Required to Implement Statutes or Executive Orders -- Commercial Items; 52.215-5: Facsimile Proposals (Facsimile offers are accepted at (562) 961-8067; 52.232-19: Availability of Funds for the Next Fiscal Year. offerors must hold prices firm in its offer for 30 calendar days from the date specified for receipt.). In addition, upon receipt of a facsimile expression interest in responding to this solicitation, a copy of FAR Clause 52.212-5: Contract Terms and Conditions Required to Implement Statutes or Executive Orders -- Commercial Items, with applicable statutes and Executive Orders marked, shall be provided via return fax.
