Description

Madigan Army Medical Center intends to procure maintenance service and repair to include all parts, labor, and travel on a Laserscope, surgical laser system, model: 813. The period of performance will be for a base year and (4) four option years from 1 September through 31 August each year Request for Quote DADA13-01-T-0075 will be available upon request to Mitch Munger Via fax (253) 968-4922 or e-mail to mitch.munger@nw.amedd.army.mil. Potential quoters should include their business size, telephone/facsimile numbers and a point of contact. This RFQ will be released on or about 25 July 2001 with a closing date of 3 August, 2001. This RFQ is being issued on an unrestricted basis. All responsible sources may submit a quotation, which will be considered. This RFQ is being issued using Simplified Acquisition Procedures (SAP) authorized in FAR Part 12 Acquisition of Commercial Items and will result in single firm fixed-priced contract. The NAICS for this acquisition is 811219 with a size standard of $5 million.****
