Description

The VA Medical Center, Louisville KY has the need for a contractor to provide readjustment counseling services to eligible beneficiaries of the Department of Veterans Affairs in the following counties in the state of KY: Adair, Breckinridge, Bullitt, Carroll, Casey, Clinton, Davies, Edmonson, Grayson, Green, Hancock, Hardin, Hart, Henry, Larue, Marion, McLean, Meade, Metcalfe, Nelson, Ohio, Oldham, Owen, Russell, Shelby, Spenser, Taylor, Washington, and Trimble. The contractor will be required to furnish approved therapy programs for set fees. Service providers submitting offers in response to the request for proposal must be physically located where a Vet Center is not present or in counties that are being serviced by a Vet Center and must be established, licensed business, with a proven track record of providing such services. Contract is for the period of October l, 2001 through September 30, 2002 with two (2)one year options, pending availability of funds. The NAICS code is 624190. The small business size standard is $5.
